Senior Mental Health Clinical Nurse Specialist for ECS

Job summary

The Mental Health Clinical Nurse Specialist for Enfield Community Services (ECS) will provide support to a cohort of patients who are being reviewed by the Multi-Disciplinary Team (MDT).

The post holder will work as part of a multidisciplinary team, maintaining close liaison with all other stakeholders in the patient's care, including their relatives. The Mental Health Clinical Nurse Specialist, in consultation with the GP, the wider MDT and / or the health psychologist will be expected to provide support to those patients who could potentially most benefit from joined up mental and physical health to optimise their health and wellbeing. They will expertly assess patients and individually plan treatment, make recommendations to improve their psychological health and give advice on monitoring and contingency regimes. They will support with patients who are struggling with adherence to medication or following agreed action plans and be a senior clinical mental health member of staff to support patients with complex physical health needs.

Main duties of the job

  1. To expertly assess patients including systemic mental health assessments, analysing signs and symptoms and other measures of function to formulate diagnosis, treatment and referral where appropriate to the identified group of patient's being assessed as part of the ECS MDT.
  2. To develop individualised evidence based care and treatment plans for the patients in close discussion with them, their relatives and the MDT staff; and with consideration of the unique presentation of psychological illness and response to treatment of people living with suspected or known mental health diagnosis.
  3. To facilitate the review of patients' medications, in consultation with the GP / health psychologist and wider members of the MDT, as appropriate, especially where there is evidence of poly-pharmacy, within own scope of practice.
  4. To attend the multi-disciplinary team meetings to represent mental health services, patients and signpost MDT and give advice on the management of complex psychological conditions and refer onto community mental health services (or other appropriate service) if required.
  5. To identify learning needs of patients and provide or facilitate the delivery of psychological training requirements.
  6. To be up to date clinically and be able to demonstrate that practice is at all times evidence based. Maintain a personal development plan and professional portfolio.

The role requires some teaching both informal and formal to educate and motivate patients to self-manage their physical and mental health needs. The role also includes regular attendance of the MDT to give advice on the assessment, management and treatment of complex psychological needs.

The role will require signposting for those already known to mental health services to the most appropriate teams and assessment and onward referrals for those not yet accessing mental health services. Service Development

To liaise closely with other professionals involved in the patients care, building relationships and negotiating to smooth the patient journey across care settings, co-ordinating care provision and acting where necessary as advocate for the patients. Ensuring adequate proactive preventative measures with residents that improve their psychological health and wellbeing in this particular population.
  2. Participate in the development of the multidisciplinary teams knowledge of specific psychological disease management and contribute to the implementation of agreed risk management strategies as part of a wider clinical governance framework.
  3. To actively promote and develop the role of proactive case management within Enfield and participate in meetings / events to represent the service and undertake any other duties that are appropriate to the role, working continuously to improve the service for users and their carers.
  4. To act as a resource to the team and Adults and Older Peoples services in general in your specific field of expertise within psychological conditions.
  5. To put in place regular audit of care pathways and critical incidents, including multidisciplinary reviews of any hospital admissions related to an unmet psychological need.
  6. To develop and implement appropriate care pathways and associated protocols of care for patients to improve clinical outcomes.
  7. To act as a link between physical and mental health services.
