We have an exciting opportunity for a motivated, forward thinking and well organised Band 3 DESMOND educator to join our friendly and supportive team.
DESMOND stands for Diabetes Education and Self-Management for Ongoing and Newly Diagnosed and is a NICE recognised national programme. The DESMOND is an established education programme designed to give people with Type 2 diabetes a good start in managing their condition.
In DESMOND courses are delivered by two accredited DESMOND Educators who will work together to facilitate group education for people with diabetes. DESMOND educators must follow a prescribed training programme as set down by the DESMOND National Programme.
You will be expected to attend DESMOND training to develop and maintain accreditation to deliver approved structured patient education programme for self-management of Diabetes in partnership with another DESMOND support worker.
This post is AfC Band 3 subject to job matching.
Main duties of the job
The Adult Diabetes Nursing service within the Wigan and Leigh Borough delivers the DESMOND programme to people with Type 2 diabetes. The post holder will, in conjunction with a Diabetes Nursing Service and DESMOND support worker/educator run DESMOND courses in the Wigan area in an educational role for newly diagnosed Type 2 diabetics.
The post holder will be expected to work flexibly to meet service requirements under in-direct supervision from the Team Lead. This will involve carrying out promotional activities for DESMOND and other public health events, liaising with a range of agencies and professionals, recording relevant and accurate information and reporting to other team members as appropriate and completing delegated administrative duties.
If you are not already an accredited DESMOND educator you will be expected to attend DESMOND training to develop and maintain accreditation to deliver approved structured patient education programme for self-management of Diabetes in partnership with another DESMOND support worker.
You will also be expected to cover short term planned and unplanned leave in line with DESMOND guidance and Trust lone working policy.

Ensure the educational space is prepared for the session and free of any potential hazards or problems, and report any problem to senior staff.
Work with the Diabetes teams administration staff to ensure courses are booked and relevant administration tasks are completed in relation to provision of DESMOND courses.
Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance;
Assess own performance and take accountability for own actions, either directly or under supervision after each DESMOND course
Effectively manage own time, workload and resources and contribute to the overall team-working of the service.
Alert other senior team members to issues of Clinical Governance issues, quality and risk; participate in reporting incidents.
Work effectively with individuals in other agencies to meet patients needs.
Maintain your DESMOND qualification and maintain your competency.
Undergo Quality Development as detailed in the QD Pathway (available from DESMOND National Office) professional development as required.
