VTE / Anticoagulant Nurse Specialist

Job summary

An exciting opportunity has arisen for an enthusiastic Band 6 Anticoagulant/VTE specialist nurse to join our team. You will work within an established team to provide safe and effective care. Previous knowledge is desirable but not essential as on the job training can be provided.

The team provides specialist care for Anticoagulant patients and Venous thrombo embolic patients within varying settings in the hospital and community.

The Anticoagulant team provides both nearside and remote patient testing for around 1000 patients requiring oral Anti coagulation therapy. The team offers advice and dosage instruction in a variety of settings, such as hospital, community, postal service, and telephone consultations. The team initiates and manages warfarin treatment, as well as initiating direct oral anticoagulants (DOAC).

The VTE team liaise with GPs across the borough taking patient referrals to the VTE unit for assessment, diagnosis, and treatment of lower limb DVTs ensuring that appropriate management is given in line with the NICE Guidance. The team also see patients who have been seen in the Emergency care centre, the wards outpatient clinics and the Same day Emergency Care for review and management of confirmed or suspected Deep vein Thrombosis and Pulmonary Embolism. The team act as a resource centre for advice from secondary and primary care and ongoing patient support.

The role is working between the two aspects of the service.

Main duties of the job

Assess patients and their medical history to determine suitability for anticoagulants.

Demonstrate knowledge and skills relevant to practice.

Provide specialist counselling and information for patients/carers in the hospital and community settings.

Undertake testing and analysis of blood results within the service, prescribe and calculate dosage of anticoagulant treatment within the boundaries of patient specific directives and national guidelines.

Communicate effectively with patients, relatives, and multi-disciplinary team.

Working with other members of the multidisciplinary team and supporting lead clinicians.
