Job summary

The Community Learning Disabilities Team are looking to recruit a Support Worker to join our friendly, forward thinking, multidisciplinary team.

The post holder will work within the Community Learning Disability Team to provide care and treatment to patients in the community setting, including the patients' homes.

The position is based in the Community division and applicants would need to be able to travel across Ashton, Leigh & Wigan to make multiple appointments daily.

To be considered for this post, youmust hold Level 2 in Health & Social Care or Care Certificate. Please do not apply for this post if you do not hold either of these essential requirements.

This is a permanent post working full time (37.5 hours) across Monday to Friday. Service hours are mostly 0900-1700 but flexibility is required to meet patient need.

Main duties of the job

  • To be a member of a Multi-disciplinary team focusing on the needs, intervention and treatment of individuals who have a learning disability.
  • The post holder will focus upon ill health prevention and health promotion, delivering treatment and behavioural management plans for individuals who have highly complex and specialist needs.
  • Key working relationships with all members of the multi-professional team, including nursing, therapists, social workers, and other agencies
  • To carry out assigned tasks involving direct care in support of a Registered Nurse bysupporting individuals in the achievement and / or maintenance of their health and wellbeing this can include health promotion for client group.
  • This may also involved when competencies achieved: blood pressures, blood samples, finger prick technique, urinalysis, bowel care, nutritional support, observations, specimen collection, prevention of pressure ulcers, moving and handling, weight - hoist, wheel chair, height, urinalysis, oxygen statistics, respirations, pulse, waist measurements,
  • Promote personal care skills of the individual
  • Carrying out behavioural and clinical observations as instructed by the registered nurse
  • Support people with learning disabilities to access generic screening services

  • Attend and participate to client focused person centred and multi-disciplinary team meetings.

  • Maintain own skills attend mandatory training.
  • Implement health promotion/ education to 1-1 or a small group as appropriate e.g. smoking healthy living etc
  • Assist in the health promotion and education along with the registered professionals.

Compliance with the Health & Safety at Work Act 1974 the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
