Trust Bank Occupational Therapist

Base is Cannock Chase Hospital and remote working.

830-1630 Monday - Friday NO weekend working

We are looking for an OT to work in the Physical Health Psychology service to support the assessment / support of patients diagnosed with Chronic Fatigue Syndrome. The service is provided to patients who are registered to GP's in South Staffordshire

Ideally, the OT would have experience / awareness of fatigue management and will work in partnership with the clinical psychologist in the team.

Main duties of the job

The OT would undertake the initial assessment of the patients and then use an established pathway to direct the next steps for the patient. There is an assessment pro forma to assist decision making. Assessments can be undertaken remotely for the majority of patients

Potential pathways post assessment are

Attendance at the chronic fatigue syndrome management group (currently delivered remotely). Most patients follow this pathway

121 interventions with OT / referral for equipment or adaptations

121 interventions with clinical psychologist

Patients may be directed to one or more of these available pathways

Ideally, we would like Occupational Therapy support in the delivery of the group as we recognise how valuable their skills are with this cohort of patient.

The Post holder will:

1. Provide a quality driven, evidence-based specialist occupational therapy assessment to patients presenting with Chronic Fatigue Syndrome.

2. Be competent as appropriate to the requirements of the role.

3. Demonstrate at all times the Partnership Trusts Values and Behaviours.

4. To assess chronic fatigue syndrome referrals and support needs. Assess and formulate, develop, implement and evaluate programmes of treatment /support for patients and carers. Takes a lead in specialist area and provides advice and support to others. The specialist care/support plans may include the following:

a. Independence/self-management and maintaining safety

b. Increased functional ability

c. Improvement in health and wellbeing including mental health

d. Interventions in condition management such as fatigue

e. Prevention and reducing of health inequalities

f. Case management/ treatment /care plans

5. Be skilled in the assessment of complex/specialist care/support needs and in the development, implementation and evaluation of care/support packages for patients and families.

6. Have supervisory, training and leadership responsibilities where appropriate, and will have some responsibility for team performance and service developments/improvements.

7. Demonstrate initiative and be creative in finding solutions to problems.

8. Work with the team leader undertaking delegated tasks in team management.
