An exciting opportunity has arisen for a NMC Registered Band 5 Nurse to work within the Home First Team (Discharge to Assess Pathway 1 based in South Staffordshire).
The Home First Team provides clinical support for patients in the community to either avoid potential hospital admissions or to support people to return home through the Discharge to Assess Pathway 1 following a period of acute care in hospital.
We pride ourselves on the delivery of quality care to community patients, many with complex needs and we actively promote the philosophy of self-care and the promotion of independence whilst offering the facilities of rehabilitation and assessment. The ethos of the team is to facilitate safe hospital discharges for patients leaving acute care and to offer them nursing support in their own home environment so they can remain in their own home and avoid unnecessary hospital admissions.
As an efficient and effective team member you will work collaboratively in a multi-disciplinary team alongside the Operational Leadership to ensure high quality care delivery to service users and provide specialist assessment and care planning in conjunction with service users and carers.
Due to the nature of this role, you will be required to work shift patterns covering a 24/7 365-day period and also be required to travel independently over a large geographical area.
Main duties of the job
As a Band 5 Nurse you will provide holistic assessment, planning, implementation and evaluation of care as part of the D2A Pathway 1 process.
As part of this role, you will be expected to:
- Act as a role model and motivate team members in working towards innovative and high standard of practice.
- Participate and support the Senior Nurse Coordinator and Clinical Lead in the development of an effective team and the development of productive working relationships throughout the Trust.
- Support the Senior Nurse Coordinator and Clinical Lead in the development of a positive and 'supportive' team culture.
- Providing public health, health promotional programs to improve health and reduce inequalities.
- Be responsible, and accountable, for service delivery to clients/patients.
