Mental Health Practitioner - Primary Care

Job summary

We are looking to recruit an experienced Mental Health Practitioner (Nurse or Social Worker) to work alongside our primary care colleagues based across 5 GP practices within the Newport and Central Primary Care Network that is based within the Telford and Wrekin area.

You will work flexible across the 5 GP practices that form part of the primary care network offering assessment, advice, information, signposting and where appropriate onward referral to the most appropriate service. You will also need to be able to offer brief psychosocial interventions at point of contact with the aim of avoiding unnecessary onward referral to secondary mental health services.

This is a clinical role and is an exciting opportunity to be at the forefront of the development of neighbourhood working, joining a small but fast-growing number of practices working in this way.

Main duties of the job

  • Formulate, contribute to, and participate in, the development and implementation of assessment and treatment in collaboration with the multi-disciplinary primary care team.
  • Carry out specialist mental health assessments, including risk and therapeutic interventions to individuals.
  • Assist in the development and implementation of care pathways, including appropriate service user assessments/referrals and models of care.
  • To take responsibility for providing holistic assessment, care and treatment to individuals referred to the Primary Mental Health Service.
  • Foster robust working relationships with all partners in particular GP's, Practice Nurses, other Primary Care Colleagues and third sector services.
  • Provision of support and advice to GPs and other primary care colleagues (such as practice nurses) to enable them to safely manage and care for people with mental health problems.
  • Contribute to the continuing improvement and development of Primary Mental Health Services.

  • A qualified/registered mental health professional.
  • At least diploma level equivalent qualification in sphere of professional practice.
  • Leadership training
  • Evidence of professional development within mental health services.
  • Mentorship Course or professional equivalent.
  • Clinical supervision training or professional equivalent.

Desirable

  • Masters level qualification in a relevant field, e.g. advanced clinical practice.
  • Additional specialist training for example Non-Medical prescribing V300.

Experience
