Job summary
We have an exciting opportunity for a NMC Registered Band 6 Nurse to join our Diabetes Specialist Service based in North Staffordshire.
You will join our team of Specialist Nurses, Consultants, Educators, andAdministration staff. The aim of the service is to provide Specialist Diabetes support and interventions within the Community and across theprimary care network through nurse led clinics and home visits.
You must have extensive and demonstrable skills specifically in Diabetes management and will ideally hold an accredited coursein Diabetes or demonstrate a keen interest in Diabetes management with a keen interest in education, teaching and mentorshipfor health care professionals and patients.
You will additionally play a key role in supporting andeducating patients, as well as community and practice staff to improve the quality of Diabetes care.
This opportunity will support professional development and enhance Diabetes knowledge. Non-medicalprescribing, whilst not essential would be advantageous.
You will be required to travel independently across a large geographical area.
Main duties of the job
As a Band 6 NMC Registered Nurse you will have detailed professional and clinical knowledge and will be a specialist practitioner with focus in the following areas:
- Diabetes care and management.
- Provide assessment of complex/specialist care/support needs and in the development, implementation, and evaluation of care/support packages.
- Provide supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance and service developments/improvements.
- Demonstrate initiative and be creative in finding solutions to problems.
- Work with the team leader undertaking delegated tasks in team management.
