Job summary
The Community Diabetes Specialist Service is looking to recruit a qualified nurse with experience in Diabetes management for the post ofCommunity Diabetes Specialist Nurse. The post holder will join the current team of Specialist Nurses, Educators and administration staff. The aimof the service is to provide Specialist Diabetes support and interventions in the community and primary care. You must be a qualified nurse with atleast 5 years post registration/ graduate experience, and hold an accredited course in Diabetes, preferably a post Graduate Diploma in a Diabetesrelated field. An ability to demonstrate experience and knowledge in education, teaching and mentorship for health care professionals and patientsis required. Nurse prescribing and experience of working in a Specialist Diabetes role is essential. Experience of carbohydrate counting, insulinpump management and the delivery of BERTIE structured education would be advantageous, although training will be provided for the rightcandidate where service delivery is required. Applicants must be able to work on their own as well as being part of the multi-disciplinary team. Youwill undertake nurse led clinics, multidisciplinary clinics and home visits. You will additionally play a key role in supporting and educating patients,as well as community and practice staff to improve the quality of Diabetes care.
Main duties of the job
oBe competent as appropriate to the requirements of the role.o Always demonstrate the Partnership Trust Values of: "People, Empowerment and Partnership".o Be an expert Diabetes practitioner with a detailed professional and clinical knowledge.o Be an advanced clinical practitioner, with experience in the management of Type 1 and Type 2 Diabetes, including insulin initiation andtitration; management of patients requiring GLP1 therapy; insulin pump management (South Division only); carbohydrate counting andstructured education for people with Type 1 Diabetes.o Be responsible for a defined population of service users in a variety of settings across the Partnership Trust including the community.o Be skilled in the assessment of complex Diabetes Specialist care and support needs and in the development, implementation and evaluation ofcare.o Have supervisory, training, and clinical leadership responsibilities where appropriate and will have some responsibility for team performance andservice developments/improvements.o Demonstrate initiative and be creative in finding solutions to problems.o Work with the Clinical Lead nurse and Operational Lead to undertake delegated tasks in team management and service improvements.
