Dementia - Assessment and Diagnosis (A&D)

Community Mental Health Nurse (Band 5)

We have two positions available: 1 post is 37.5 hours per week and 1 post is 35 hours per week

We are advertising two posts for Community Mental Health Nurses to work within our Assessment and Diagnosis team. • Liaise with internal and external teams
  • Support those in post-diagnostic period where there is a specific need and work in conjunction with our Specialist Dementia Nurse Practitioners
  • Involved in service development
