Job summary
South Shropshire Mental Health Service is looking to recruit a part time NMC Registered Community Mental Health Nurse to work within Community Mental Health in South West Shropshire at Band 5. The post is suitable for newly qualified nurses and preceptorship will be offered. The post is based in Ludlow but there is an expectation that cover will be offered across the South of Shropshire.
The hours of work will be 9-5 week 1 : 22.5 hours over 3 days, Week 2 : 30 hours over 4 days, actual days of work can be negotiated at interview but the service operates Mon to Fri 9-5.
We are looking for someone who has a passion for community work, ideally with experience, relevant knowledge and a commitment to working with adults with mental health needs. There will be an expectation that the post holder will act as a key worker offering recovery focused Interventions, provide supervision to support workers and work in collaboration with the multi-disciplinary team. Experience of providing Brief Interventions would be an advantage but full training can be offered to develop the essential skill base.
A valid UK driving license plus the use of your own car for business purposes is essential due to the large rural nature of our catchment area.
Main duties of the job
Act as a keyworker for an identified caseload of service users , which includes assessing, care planning in conjunction with service user and carer, evaluation of individual treatment plans (Single Assessment Process).
Administer and ongoing monitoring of mental health medication, complying with relevant Trust procedures and NMC Guidance.
Assess patient's individual care needs, develop, implement and evaluate programmes of care.
Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including; pharmacological and psychologically informed interventions.
Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
