Job summary
The aim of a virtual ward is to reduce length of acute hospital stay and/or avoid hospital admission; this leads to quicker recovery and reduces disruption to patients', andtheir carers' lives. This means we can proactively manage people living with frailty athome, and for those that have a sub-acute episode we can improve a patient'sexperience and outcomes. In addition, this means we have more space in our hospitalsfor acutely unwell patients and reduces the pressure on urgent care.
Please note this role is patient facing in the community
Applicants will need to hold a full driving licence and access to vehicle for work, as this role is required for the role.
Main duties of the job
As a Healthcare Support Worker in this team, you will contribute to the assessment and implementation of programmes of treatment care/support for individuals, families and carers under the guidance of a registered practitioner.
The care/support plans may include the following:
- Promoting independence/self-management and maintaining safety
- Increased functional ability
- Improvement in health and wellbeing including mental health
- Interventions in disease/condition management
- Prevention and reducing of health inequalities
- Admission avoidance
- Early supported discharge
- Contribute to case management/ treatment care plans
- Palliative and end of life care
- Venepuncture
