Job summary
We are offering an exciting opportunity for a Band 6 Community Mental Health Practitioner to work within our Community Mental Health Service. The service offers support to people with severe & enduring mental illness as well as a comprehensive brief intervention system for people requiring a specialist mental health short term service.
The service has bases at Oswestry and at Market Drayton and covers a geographic area across north Shropshire. You will need to travel independently over a large geographical area and will require suitable transport.
This service is expanding, and it is an exciting time to join the service as a highly skilled NMC Registered Nurse or Social Work England Registered Social Worker.
North Shropshire Community Mental Health Team is a multi-disciplinary service providing assessment and intervention for adults experiencing complex emotional and mental health difficulties as part of the Adult Mental Health Service.
Main duties of the job
As a Mental Health Community Practitioner you will be expected to undertake holistic comprehensive assessments and formulation of complex clinical presentations, including risk assessments and risk management.
The role will involve:
- Contributing to the service Duty System, where you will be required to make telephone triage assessments, consider the urgency of the callers referral and make immediate decisions with the callers to ensure safety planning whilst discussing an initial care pathway / signpost for the person.
- There will be a requirement to undertake the role of named worker for a caseload of service users.
- Participate in both delivering and receiving a range of clinical supervision and professional and clinical development within the pathway.
- Provide consultation, support and specialised higher level knowledge regarding mental health to colleagues, stakeholders, partners and VCSE colleagues working within a range services, including primary care, Local Authority and other areas of the Trust where identified as appropriate.
- Having an embedded understanding and experience of Trauma informed Practice would be an advance. Having an understanding of societal discrimination, health inequality and the impact would equally be of high value in this role.
- As a social worker there is no Statutory requirements within this role. We would support AMHP & BIA career training and have partnerships with Shropshire Local Authority.
