Job summary

We have an opportunity for a highly motivated and autonomous Specialist Occupational Therapist to join our well established Discharge to Assess (D2A) pathway 2 Service. Following successful service development, we are developing our D2A pathway 2 service giving you the opportunity to help drive developing and implementing high quality care pathways and MDT training plans. Actively contributing to building a culture of research and development, with strong links to our local community supported by an invested interest in improving our bed-based services.

Main duties of the job

This is a unique opportunity to work in a transformative and innovative way with the autonomy to shape the future of D2A within North Staffs. Working in co-production with our service users and local D2A networks; with the overall aim being to improve the health and wellbeing of the population served.

Experience

Essential

  • oRelevant experience to the role in a variety of specialties including neurology, orthopaedics, older peoples rehabilitation, falls management, respiratory care, musculoskeletal including assessment and management of various conditions within the area of clinical speciality.

  • oExperience of risk management including risk assessment, action planning & evaluation
