Job summary
Are you a Qualified Mental Health Nurse that has an interest in working in a community team but would like some support to see if this is the right career path for you or are you moving to the South Shropshire area. If so South Shropshire Mental Health Service has an opportunity for a fixed term Band 6 Nurse to join the team until May 2027. The post holder will work from the Bridgnorth team base and cover across the South East of Shropshire.
The post offers the successful applicant the ability to learn the skills of managing a community caseload with support from the wider MDT. The role also offers the opportunity to learn how to assess a person's mental health needs, make holistic plans of care involving the service user and their family/carers and evaluate the care at regular intervals to help the person work towards Recovery. The role also involves the ability to learn the skills of managing day to day calls to the team via the Duty system and making clinical judgements on the correct pathway for a person's needs.

The post is community based visiting people in their homes hence Driving and use of a car is essential for this role
Main duties of the job
- Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in collaboration with service user.
- Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
- Assess patient's individual care needs, develop, implement and evaluate programmes of care utilising patient related outcome measures(PROMs)
- Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including; pharmacological and psychologically informed interventions.
- Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
- Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
- Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
- Assist in ensuring that the aims and objectives of the ward/department (as set by the responsible clinician) are fulfilled and to identify factors which may inhibit these from being achieved.
- Work within the NMC Code of Practice and professional guidelines.
