Job summary
We are offering an exciting opportunity to join the Integrated Complex Assessment Team Discharge Facilitator based in Cannock, however; will be working within community care homes in South Staffordshire (Cannock, Walsall, Stafford)
You will work closely with multi-disciplinary teams under the direction of the Patient Flow Team and will be responsible for providing effective support to assist in the delivery of safe and effective patient transfer and tracking within Discharge to Assess Services whilst gathering and collating collateral histories and information regarding current care needs.
You will prioritise own workload to ensure effectiveness and efficiency in delivering objectives. Being proactive and able to demonstrate good organisational skills and excellent communication skills both in verbal and written correspondence.
This role is for 30 hours per week, working 8am-6pm.
Main duties of the job
As discharge Facilitator you will attend Multi-Disciplinary meetings, sharing and recording information to enable accurate assessment and robust flow through the D2A beds escalating any blocks or issues to the Flow Co Coordinator.
As part of this role, you will be expected to:
Gather and record information to enable accurate and individualised patient assessments to facilitate timely discharge of patients to their own home or an alternative care facility.
Be responsible for the clerical coordination of the discharge process from the point of admission until discharge whilst building robust relationships with patient's families and the multi-disciplinary team.
Liaison with families and professionals including external agencies to provide timely updates and discharges including the booking of transport.
Work closely and at pace, assisting other members of the team to meet deadlines, sharing workload and providing cover for absences where necessary, attending meetings and updating CDS.
Working with health and social care staff and an understanding of community Home First services alongside knowledge of the Care Act 2014 and the principles of Discharge to Assess (D2A) would be desirable
