Job summary
A fantastic opportunity has arisen for aCommunity Support Worker to join the Assessment and Diagnosis team within the Dementia Assessment and Support Service.
We are in an exciting period of change due to the Dementia Transformation taking place across Shropshire, Telford & Wrekin.
This community-based role will involve supporting the triage and initial contact with service users and their families, working in the community with our qualified staff and offering post-diagnostic interventions to those who are diagnosed with a dementia.
The service can be very fast paced and therefore the ability to work efficiently under pressure and respond to high service demand is essential. Additionally, the post holder will need to be confident to work autonomously and organise their time effectively. You will be highly motivated, have excellent communication skills and work well as part of a team, they will also have determination and drive committed to working holistically to improve the quality of life for people with dementia, their family and carers and also demonstrate a range of therapeutic interventions.
A good positive attitude towards this specialised client group needs to be demonstrated and also the aptitude to the contribution of continual development of the service incorporating evidence-based practice.
The post holder will be expected to adopt a blended working approach.
Main duties of the job
You will be responsible for the provision of planned care working in partnership with service users. You will work collaboratively with other Trust services to provide seamless care for service users and their families within the Dementia Assessment and Support Service.
- Ensure the delivery of high quality care through and demonstrate high personal standards in clinical practice.
- See service users as appropriate, to carry out agreed interventions under the supervision and guidance of the lead professional.
- Work with multi-disciplinary team
- To participate where appropriate in robust risk assessments and risk management plans under the supervision and guidance of the multi-disciplinary team and support the team to practice positive risk taking where appropriate to improve quality of life, well-being and maximise occupational performance/independence.
- Under the supervision and guidance of the multi-disciplinary team to deliver a range of non-pharmacological interventions following specific assessments to enable understanding of any expression of an unmet need and to maximise an individuals abilities.
- You will be expected to undertake senior healthcare support worker competencies including physical health competencies
