North D2A Moorlands and Newcastle Hospital Team. Band 5 Senior Social Care Assessor

37.5 hrs (Full Time Permanent)

Going forward this post may involve some weekend working on a rota basis

An opportunity has become available in the North Unplanned care service for a permanent Band 5 Senior Care Act Assessor. We need an experienced, resilient and dynamic individual to join this knowledgeable and enthusiastic team.

Main duties of the job

The work is varied and challenging and will focus on pathway 2 of the D2A service. You will be supporting and assessing people within a rehab focused bed-based setting, following an acute hospital stay. The core work will be undertaking statutory assessments within complex situations, so knowledge of the Mental Capacity Act 2005 and Continuing Health Care legislation is essential.

The successful applicant will be part of a multi-disciplinary team including nurses, physiotherapists, occupational therapists, discharge facilitators and support staff within the Haywood hospital and private residential and nursing homes. The team also works closely with a number of external partners such as the CCG/ICSs and Staffordshire County Council. The work is on a short term basis and cases are usually closed within 12 weeks; this means the successful individual will need to be able to work at pace and be able to move between competing priorities.

  • Level 3 Diploma in Adult Care or Lead Adult Care / Social Care Worker
  • Level 4 Diploma in Adult Care or Lead Practitioners in Adult Care Level or willing to complete / achieve this if a Level 3 qualification is already held

Desirable

  • Work based supervisor training (Level 3)

Experience

Essential

  • The ability to demonstrate a commitment to working in partnership with adults, carers, professionals and voluntary agencies, to provide a comprehensive service which meets the needs of the adults in line with current legislation and directorate, local authority & Trust policies
  • Experience of working in a team, providing formal support and supervision to junior colleagues, incorporating good interpersonal relationships and the ability to actively contribute to the implementation, development and review of an individual's care and support plan
  • Experience of working within the specialist area
  • Experience of working in an NHS/Local Authority setting where MDT and specialised practice associated to social work and social care is required
  • Experience in dealing with complex casework involving family members, carers, communities and other organisations
