Job summary

The post holder will be part of the Haywood Hospital Therapies Portfolio providing physiotherapy delivering specialised stroke rehabilitation, focusing on multi-disciplinary, goal driven and person centred approach, on an inpatient specialised stroke rehabilitation ward. This service works over 7 days. The role sits within a wider therapy team covering the stroke rehabilitation pathway and involving an element of joint working across the pathway as the service requires. Alongside our multi-professional teams, our aims are to deliver excellent, evidence-based services working to national standards and service specifications. Your role will actively assist clinical & service Leads, in partnership with colleagues, to help shape and develop services within current and future transformation plans to support ICS's, changing service user demographics and increases in multi-morbidities. You will need the following key skills and experience:

Qualified Physiotherapist with HCPC registration, at least 2 years' experience post-graduation

Experience of working in an acute or rehabilitation ward setting

Experience of working with people with neurological diagnoses

Skilled in the assessment of complex/ specialist care within the stroke speciality, and the development, implementation and evaluation of rehabilitation programmes

Have supervisory, training and leadership responsibilities

Main duties of the job

Be competent as appropriate to the requirements of the role.

Be a Specialist Physiotherapist and have a detailed professional and clinical knowledge within the specialised stroke rehabilitation service. The post holder will be a specialist practitioner and will primarily be based within the inpatient stroke rehabilitation team

Be responsible for a defined population of service users in a variety of settings across the Partnership Trust including the community and hospitals.

Be skilled in the assessment of complex/specialist therapeutic/care/support needs and in the development, implementation and evaluation of therapy/care/support packages.

Have supervisory, training and leadership responsibilities where appropriate and will have some responsibility for collective leadership, team performance and service developments/improvements.

Demonstrate initiative and be creative in finding solutions to problems seeking innovation and service development as core elements to role.

Work with the Clinical Leads / Management undertaking delegated tasks in team management

To undertake shift patterns / 7 day working within service specification as required across Trust sites including community working and lone working

To establish and maintain effective working relationships both within the Partnership Trust and with external organisations in order to meet service requirements and support and demonstrate the Trusts equality values.

To assess new service user referrals and care/support needs. Assess service users and formulate, develop, implement and evaluate programmes of treatment /care/support for individuals, families and carers. The specialist care/support plans may include the following:
