Job summary

We have a vacancy for a Band 6, Level 2 Social Worker to join the East Staffordshire Discharge to Assess Team. The Discharge to Assess team is a multi-disciplinary team, the social care staff carry out statutory assessments for patients that have been discharged from hospital that have care and support needs as an ongoing requirement. The social care team work very closely with allied health professionals to achieve the best possible outcome for these patients, promoting independence whilst striking the balance of working within timescales of this service. This is a fast paced team, so we are looking for an enthusiastic individual who has experience of prioritization and good time management skills.

Unplanned Care South East service includes the Discharge to Assess (D2A) East Staffordshire and also bed based facilities in Lichfield and Tamworth (BLT) and East Staffs and surrounding areas. You will work closely with other Multi-Disciplinary team members including Social Workers, Social Care Assessors, Nurses, Therapists (Occupational Therapists and Physiotherapist), and Home First Teams and external agency partners. The successful candidate will have a flexible approach and be confident to forge partnership working relationships with the Multi-Disciplinary Teams across the various teams, hospitals and community sites.

Main duties of the job

You will have the skills, knowledge and experience to work autonomously and to assess Staffordshire residents. The successful candidate will require to have extensive knowledge of The Care Act 2014, a good understand of discharge to assess process and experience of conducting s.42 investigations and knowledge of the Continuing Healthcare funding process.

You must be enthusiastic, highly motivated and be able to work under pressure with a strong commitment to promoting timely, safe , holistic and person centered assessments. Supporting individuals in the community to prevent readmissions and assess need for ongoing care and complete timely reviews to support individuals remaining at home whilst promoting ongoing independence.

The successful candidate must be a confident practitioner, team player with excellent communication and IT skills.

You will be expected to work between the hours of 08:00 - 17:00
