You will be part of a team striving to achieve the best in clinical care. The service will deliver a pathway of care in accordance with the latest evidence-base, which puts the service users needs at the centre of the service.

We cover a 7-day service 08:00 - 18.30hrs, however as a Band 3 Health Care Support Worker you would be working 09.00-17.00. Due to the nature of community working, ability to travel across the patch to provide care to patients is essential; access to a car during working hours is required.

Main duties of the job

To adhere to the Care Certificate standards and all other relevant guidance from the Partnership Trust. To be aware of the Nursing & Midwifery Council's (NMC) guidance/codes of conduct.

To establish and maintain effective working relationships both within the Partnership Trust and with external organisations in order to meet service requirements and support and demonstrate the Trust's equality values.

To contribute to the assessment and implementation of programmes of treatment care/support for individuals, families and carers under the guidance of a registered practitioner. The care/support plans may include the following:

  • Promoting independence/self management and maintaining safety
  • Increased functional ability
  • Improvement in health and wellbeing including mental health
  • Interventions in disease/condition management
  • Prevention and reducing of health inequalities
  • Admission avoidance
  • Early supported discharge
  • Contribute to case management/ treatment care plans
  • Palliative and end of life care
  • Specific skills as listed in person specification

To consider each service user's personal circumstances and preferences in order to implement and adjust programmes of care and intervention.

To maintain and develop skills and knowledge to deliver a range ofinterventions under the guidance of a registered practitioner and set an example of good practice to other team members including skill sharing.
