We have an exciting and rare opportunity to join the Stone Rehabilitation Service based at Stone Rehabilitation Centre on a full time basis. The team is dedicated to delivering excellent customer service and aims to improve function, independence and promote self-management through therapy programmes and/or equipment provision.
Main duties of the job
A multi-disciplinary approach is taken to assess and treat individuals referred to the service. You would be working alongside Physiotherapists, Occupational Therapists and Rehabilitation Technicians to provide patients with rehabilitation goals and treatment plans. The successful applicant will be required to:
- Support the implementation of intervention/therapy programmes developed and agreed by qualified staff.
- Train patients and their carer's in the safe use of assistive aids and equipment
- Liaise with community and voluntary services, signposting where appropriate.
- Plan, deliver and evaluate delegated rehabilitation within clear protocols.
- Deliver health promotion and self-management advice.
- Support in the completion of patient records, ordering equipment and data cleansing.
- Maintain clear and accurate patient records.
- Contribute to the patient experience programme.
- Demonstrate the vision and values of the organisation.
- Have recent experience working in the NHS.
A car driver is essential for this post due to the nature of our work.

- To contribute to the assessment and implementation of programmes of treatment care/support for individuals, families and carers under the guidance of a registered practitioner. The care/support plans may include the following:
- Promoting independence/self-management and maintaining safety

Supervision/Administration
- To participate in regular supervision
- To contribute to the process of induction for new team members and demonstrates duties to others as required.
- To identify own training/educational needs as part of the supervision and appraisal process in conjunction with registered practitioners. Attains and maintains an agreed level of competence through ongoing training and development.
- To collect and input statistical information as required.
- To participate in and support the team in undertaking clinical audit.
- To actively promote the implementation and upholding of policies, procedures, guidelines and directives, along with relevant statutory measures and governmental directives
- To assist in general administrative duties as required including dealing with messages, photocopying, filing, receptions duties and appointment management.
- The post holder may be required to order stock through the E-Procurement system and monitor stock levels for their own area
