Job summary

This is an exciting opportunity to work as part of the expanding Health and Justice Reconnect team in Cheshire.

The aim of the Reconnect service is to reduce health inequalities and the potential for reducing re-offending. To do this, service users are assessed whilst they are in prison or an immigration removal centre, if the person has identified health needs and will struggle to engage in services in the community, Reconnect will support the person on their release into the Cheshire area.

As part of this support there will be a requirement for the post holder to attend the following prisons, HMP/YOI Styal, HMP Risley and HMP/YOI Thorn Cross to build a rapport with the service user and participate in release planning with service users.

Due to the requirement to attend the prisons, additional vetting will be applied for by the Trust as part of the recruitment process, however this can be discussed further dependent on individual need.

The Health & Justice service is not itself a treatment service, but it is an identification, assessment, and engagement service to support people to access and engage in services to address their identified vulnerabilities and health needs. Cross-cover to the Health and Justice Community Outreach team within Cheshire may be required if necessary, offering a similar service to those who have had contact with the Police or Court system.

Main duties of the job

Working with service users referred to Health & Justice and the individual's carers, helping to promote self-determination, choice and autonomy.

To effectively establish a therapeutic relationship with service users and relatives / carer.

Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.

Assess patient's individual care needs, agree, and refer on to appropriate agency. Involve the individual in the planning and prioritisation of their activity where possible. Provide short term intervention to encourage individual to engage with services and attending their initial appointments.

Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.

Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
