Job summary

Our North Unplanned Care Service are looking for a highly skilled, resilient and dynamic individual to join our team as a Band 6 Level 2 Social Worker.

Based at the Haywood Hospital, the work is varied and challenging and will focus on pathway 2 of the Discharge to Access (D2A) Service. You will be supporting and assessing people within a rehab focused bed based setting following an acute hospital stay.

This is an ideal opportunity for individuals that can demonstrate advanced skills and practice with a clear desire to work within an Unplanned Care setting. The work is on a short term basis and cases are usually closed within 12 weeks; this means that you will need to be able to work at pace and be able to move between competing priorities.

You will also be enrolled onto our Social Worker Learning Academy (SWLA) which is aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA 'stage' (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. There will also an opportunity to train as a Practice Educator.

As part of our commitment to Flexible Working, flexible/blended working is supported and following a period of induction staff will work mostly from home to complete documentation, with regular face to face visits as required.

Main duties of the job

The core work will be undertaking statutory assessments in complex situations and will have a key role to play in supporting the delivery of the Personalisation and inclusion Pathways for Adults and Carers ensuring legal literacy, Strength and asset based practice, personalisation and social inclusion is embedded and applied effectively throughout their work.

The role will involve assessing; accessing; and implementing a range of social care services, and providing guidance/advice and information for adults with acute or increasingly complex long term physical health, mental health needs and for older people and carers. As part of this role you will be expected to:

  • Work as part of a multi-disciplinary team including nurses, physiotherapists, occupational therapists, discharge facilitators and support staff within the Haywood hospital and private residential and nursing homes.
  • Working closely with a number of external partners such as the ICB and Staffordshire County Council.
  • Have in-depth knowledge of Mental Capacity and Continuing Health Care legislation is crucial.
  • Knowledge of safeguarding adults will also be an advantage.
