We have an opportunity for a highly motivated Rehabilitation Assistant to join our well established and Award-Winning nationally recognised community Stroke Service.
Main duties of the job
Following successful service development, weare embarking on Phase II of our new Integrated Community Stroke Model (ICSS). Giving you the opportunity to help us develop and implement high quality care pathways and MDT training plans. Actively working with us to build a culture of service development, with strong links to our local community supported by an invested interest in improving Stroke Rehabilitation. This is a unique opportunity to work in a transformative and innovative way helping us shape the future of Stroke Rehabilitation within North Staffs. Working in co-production with our service users and stroke networks; with the overall aim being to improve the health and wellbeing of the population served.

- Qualifications and Credit Framework QCF/NVQ/ Care certificate / Apprenticeship Level 3 or equivalent in related subject or evidence of comparable academic study.
- Evidence of/willing to undertake training relevant to the post.
Desirable
- Care Certificate is completed in all 15 areas within 12 weeks (pro rata) of commencement of contract.
Experience
Essential
- oWorking as part of a team oWorking in a care environment oSupporting or contributing to the management of individuals with disabilities.
Desirable
- Relevant placements/experience to the role including supporting or contributing to the management of individuals with compromised functional ability
