Shrewsbury Central East Community Mental Health Service provide assessment and evidence based interventions for people who presenting with complex mental health difficulties that are significantly impacting on their daily life alongside our partner organisations.
An exciting opportunity has arisen to recruit a Band 7 Mental Health Nurse Practitioner / Non Medical Prescriber within our service.
We are seeking a dynamic, highly motivated individual who is a registered non-medical prescriber (NMP) with experience ideally of working within a community setting. As a registered professional, you will be committed to providing a high-quality service and will support to improve service user access to care and treatment.
Ideally you will have experience with working with service users with a range of mental health diagnosis / formulations. You will be working as an autonomous prescriber and will effectively manage a dedicated caseload.
Main duties of the job
- You will work predominantly work with service users who are registered within Central East of Shrewsbury PCN, as a non-medical prescriber working autonomously on completing assessments and reviews for service users deemed appropriate for this service.
- You will carry out assessments, reviews and associated prescribing for service users following agreed practice guidelines as part of a multi-disciplinary team and to provide on-going care reviews.
- You will carry out reviews of service users who are having medication prescribed in primary care and need an annual review for this to continue.
- You will be responsible for the development, implementation and review of care planning and risk management.
- You will deliver a high quality clinical service to Service Users, their Families and Carers in the Community.
- You will demonstrate an attitude which respects the core values of the organisation and the principles of Recovery.
