Are you an experienced social worker, occupational therapist or community mental health nurse who would like the opportunity to be part of a developing and innovative integrated Community Mental Health Service?
Telford and Wrekin CMHS provide assessment and evidence based interventions for people who presenting with complex mental health difficulties that are significantly impacting on their daily life alongside our partner organisations.
The successful applicant will need to encourage effective engagement of this cohort of people, utilising excellent communication skills, whilst maintaining compassion and hope through every contact. We also keen to incorporate a trauma informed psychological approach which aims to increase people's independence, working towards an improved quality of life.
There is an emphasis on team based multi-disciplinary decision making which lends itself to creating a supportive environment for staff. We also value a culture of active service and personal development for all staff.
We are looking to focus this particular role on working alongside the wider team, as well as our inpatient colleagues, to improve people's experience as they move from being in hospital back to their communities.
We are particularly interested to hear from social workers who may be interested in supporting people with this transition.
Main duties of the job
To work within the Telford and Wrekin adult community mental health service;
- Delivering a high quality clinical service to patients and their families.
- Managing a complex caseload
- Responsible for clinical and caseload supervision of other team members where identified.
- Responsible for providing the assessments and interventionsinto the clinical pathway.
- Delivering assessments and interventions, in accordance with an agreed care plan, to an identified group of people experiencing complex mental health related difficulties.
