Job summary

How would you like to make a difference to the outcomes of children in care within a dedicated, well established team?

We are looking to recruit motivated practitioners with initiative and drive to join our friendly and enthusiastic team and contribute to the further development of our service.

We support approximately 2500 children in care across the areas of Staffordshire and Stoke on Trent. We undertake the statutory health assessments for these young people, ensuring that they remain the focus of the assessment and that their health needs are met.

Experience of working with children in care is essential.

Main duties of the job

  • Undertake health assessments for looked after children and complete summaries and health care plans with SMART action plan and provide written/electronic reports as required.
  • Following national and local guidelines, plan, organise, deliver and evaluate comprehensive health assessments to assess and formulate the complex needs of a looked after child or young person using evidence based knowledge to develop health plans toward improving the long term health outcomes for looked after children
  • The post holder needs to be an autonomous practitioner who develops and embraces a flexible range of working regarding the way services can be delivered to children and young people who are looked after toward achieving positive outcomes. Post holder to be available for the occasional evening work.
  • Ensure that the health of Looked After Children is positively promoted, encouraged and supported. Empower children and young people to take responsibility for their own health and wellbeing.
  • The role will require travelling across the geographical area of Staffordshire and at times slightly beyond.

Job responsibilities

  • Work closely with all health professionals involved to ensure a comprehensive health assessment for children and young people is undertaken.
  • Develop and undertake age appropriate health promotion advice and act as a resource providing health advice to children, carers and professionals.
  • Establish robust lines of communication relating to health care between service users, professionals and agencies.
  • To attend as appropriate Annual Reviews as needed for Looked After Children to inform and advise on the child/young persons health needs.
  • Ensure those children who are Looked After by Other Local Authority (OOA) but are placed IN the boundaries of MPFT have their health needs met in a timely and consistent manner ensuring good communication with those placing Authorities.
  • To establish and maintain effective links across organisational and inter-agency boundaries.
  • To provide professional, advice on the assessments and participate with the Team Leader in training on issues relating to the health of Looked After Children to carers and other agencies.
  • To model a good standard of health assessments/report writing for Looked After Children and to support other health staff in undertaking these.
  • To communicate and receive highly complex and sensitive information in relation to Looked After Children.
  • Discuss issues of this nature with young people and their carers.
  • To be able to plan and organise workload and work calmly under pressure. Maintain work diary/calendars.
  • To support the Team Leader and be involved in the planning, development and implementation of a variety of training i.e. for carers/professionals to raise the profile and standards of health care for Looked After Children.
  • Contribute to the ongoing reporting and evaluation of health services for looked after children.
  • Demonstrate ability to manage own workload including data collection, maintenance of records and ensuring that reporting mechanisms are in place and working effectively.
  • Be able to communicate health information accurately in both a verbal and written form to health colleagues, social workers, families, carers and service users that conforms to policies around consent, confidentiality and information sharing agreements between agencies.
  • Be skilled at communicating with children and young people across the age ranges from diverse cultural and religious backgrounds, children and young people with learning disabilities or who have some degree of communication difficulties.
  • Share information within health and other service providers e.g. Social Services in accordance with data protection policies, data exchange agreements and records management policy.
