As a Registered Mental Nurse with current NMC registration you will have post qualification experience working with Asylum Seekers and Refugees, the challenges they overcome and the resilience and resources they have acquired. With strong communication and organisational skills and a flexible approach, you will work collaboratively with patients and key partners to facilitate the best possible patient experience and outcomes.

You will demonstrate the ability to effectively and efficiently assess, plan, provide and evaluate evidence based nursing care for patients.

You will demonstrate strong clinical reasoning and prioritisation skills and be able to work effectively, both independently and as a member of a team.

You will have a commitment to professional development and contribute to a culture of continuous improvement.

You will organise and prioritise your own day- to-day work tasks and activities and delegate tasks to junior colleagues.

Job description

Job responsibilities

  • Take responsibility, with supervision from the senior staff/operational leads, for elements of the specialist screening assessment and care planning in conjunction with service user and carer, evaluation of individual treatment plans and discharge planning.
  • Be responsible for maintaining quality standards, including the auditing, monitoring, and reviewing of practice in line with current clinical guidance practice and policy.
  • Assessing patient's individual care needs, develop, implement, and evaluate programmes of care.
  • Deliver a range of screening and advisory interventions in accordance with the agreed plan of care and Pathway guidance including signposting or referral to GP for medication / physical health needs where required.
  • Undertake, where appropriate, risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life preventing social isolation and empowering service users.
  • Maintain an effective reporting system by observing and reporting verbally and in writing on patient condition
  • Utilise agreed outcome measures on the mental health screening assessment tool and RiO to review the effectiveness of interventions, as part of the care planning and evaluation process.
  • Support in the provision of formal and informal training of pre/post registration students, carers and service users and volunteers/ facilitators in Community Groups and Charities.
  • Take an active role in the development of the service and maintain clinical governance processes at team level.
  • Assist in developing and maintaining the aims and objectives of the service and to identify factors which may inhibit these from being achieved.
  • The post holder will be required to work within the NMC Code of Practice and professional guidelines.
  • Maintain an up-to-date level of professional competence within the specific environment.
  • Assist in the implementation of evidence-based nursing practice and participate in research and other projects as required.
