Job summary
We are looking for an enthusiastic, forward thinking, passionate and motivated Band 5 Mental Health Nurse or Allied Mental Health Professional to join our Bee U Crisis intervention Home Treatment Team. This role is a development post and upon successful completion of the development process you will be appointed to a Band 6.
You will work closely with the Operational Lead, Clinical Lead and Band 6's to ensure safe and effective working across the team. The team work 24/7, therefore shifts are part of the working practice.
Main duties of the job
- Provide specialist assessment and care planning in conjunction with service users and carers, evaluation of individual treatment plans and discharge planning
- Contribution to the delivery of home treatment through nursing care to an identified community client group and within a multi-disciplinary team.
- Support for and facilitation of, early discharge from hospital.
- This is a 24 hour service which will involve shifts including weekends and nights.
- To provide an active involvement in the assessment, development, implementation and evaluation of service user care which focuses on ensuring that service users are supported to remain in their own homes wherever hospitalisation can be safely and appropriately avoided.
- Assess patient's individual care needs, develop, implement and evaluate programmes of care.
- Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including; pharmacological and psychologically informed interventions.
- Complete and contribute to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
- Maintain an effective reporting system by observing and reporting verbally, electronically and in writing on patient condition
- The post holder will be required to work within the NMC/HCPC Code of Practice and professional guidelines
