Main duties of the job

Band 6 Community Mental Health Nurse role

37.5 hours per week - initial two year contract with West Mercia Probation/Police

Agile working: Mixture of home working, community visiting and some time at West Mercia Probation and Police Stations - Telford, Shrewsbury, Worcestershire. The successful applicant can select the best base for their work/ life balance.

This post is ideal for someone interested in working in partnerships across boundaries, offering focused brief interventions and adding to their skills in the criminal justice area of practice.

Integrated Offender Management (IOM) was introduced in 2009. Its aim is to bring a cross-agency response to crime and reoffending threats faced by local communities. Service users will be jointly managed by Police, Probation and other partner agencies; to reduce reoffending and make communities safer. The nursing role has support and supervision from the Forensic community team and will have a partner I.O.M nurse in West Mercia, in addition to being part of the virtual IOM team.

  • Offer mental health assessment, support and treatment for the Integrated Offender Management I.O.M population

  • Refer to primary mental health, secondary mental health services where care coordination is needed.

  • Develop close partnerships, training and support across West Mercia police, Probation and the third sector.

This role sits within the criminal justice teams and fills the gap between short term criminal justice services such as Liaison and Diversion and Primary and secondary mental health Teams.

The main duties include:

Assessment and care planning in conjunction with service, carer, probation and West Mercia Police.

  • Participate in and contribute to weekly IOM case management meetings, ensuring that colleagues from other agencies are kept fully informed of the status and progress of IOM individuals on caseload.
