Main duties of the job

Haywood Hospital Therapies Portfolio prides itself on the delivery of quality care to all patients (ward and community based), many with complex needs. We are looking for enthusiastic Therapy Support Workers to join our teams working in a variety of settings including D2A (hospital & community beds) & Integrated Stroke Community Team and across other services as needed. We are currently offering a range of permanent and temporary posts to support with current service demand.

These are dynamic roles that requires collaborative working as a system supporting bed flow, managing timely discharges and post discharge supporting longer term adaptation and resettlement back into the community. The overall aim being to improve the health and wellbeing of the population served.

We are looking for highly motivated, proactive, enthusiastic, good team workers with excellent organisational and interpersonal skills. The ability to prioritise tasks together with effective communication is essential. You should have a methodical and organised approach to work and be able to work under your own initiative once fully trained. The nature of the service will be determined by local service specifications.

You will be required to support current service pressures and be flexible in approach across our 7 day services. Community working requires essential car driving on daily basis and lone working in line with service / Trust Policy.

Haywood Hospitalrehabilitation services are a highly complex area of the care and the post holder will act as a professional and be competent as appropriate to the requirements of the role:

  • Demonstrate at all times the Partnership Trust values and behaviours
  • To adhere to the Care Certificate standards and all other relevant guidance from the Partnership Trust. To be aware of the Health and Care Professions Councils (HCPC) and relevant professional specific guidance/codes of conduct.
  • To establish and maintain effective working relationships both within the Partnership Trust and with external organisations in order to meet service requirements and support and demonstrate the Trusts equality values.
  • Work with Registered / multi-disciplinary team in the implementation of therapeutic programmes and provide regular feedback to support efficient and effective rehabilitation
  • Support Registered / multi-disciplinary team in the preparation and delivery of specialist programmes in line with national clinical guidance / evidence based practice
  • Support in delivery of administrative duties as appropriate to service
  • Participate in quality improvement and / or service improvement & development tasks
  • Take an active role in health promotion and service promotion activities and initiatives
  • To contribute to the assessment and implementation of programmes of treatment care/support for individuals, families and carers under the guidance of a registered practitioner in accordance with the care/ support plan and agreed goals. The care/support plans may include the following:
    • Promoting independence/self-management and maintaining safety
    • Increased functional ability
    • Improvement in communication, nutrition & health and well-being including mental health
    • Interventions in disease/condition management
    • Prevention and reducing of health inequalities
    • Contribute to case management/ treatment care plans

  • To consider each service users personal circumstances and preferences in order to implement and adjust programmes of care and intervention.
  • To maintain and develop skills and knowledge to deliver a range of interventions under the guidance of a registered practitioner and set an example of good practice to other team members including skill sharing. To provide rehabilitation to service users within specified areas.
  • To maintain sensitivity at all times to the emotional needs of the service user and their carers offering reassurance and empathy. This may be about the nature/prognosis of their condition and implications of the same.
  • To work with individuals and carers who may find it difficult to engage with the service or aspects of the service resulting in uncooperative or challenging behaviour that has to be managed.
  • To apply skills, knowledge and competences to recognise any significant change in the holistic needs of the service user/carer and feedback to the registered practitioner.
  • To maintain contemporaneous and accurate records as required by the Partnership Trust and in line with HCPC standards.
  • To make resources for use in therapy/care taking direction from registered practitioners as required.
  • To use problem solving and innovation to deliver a high standard of intervention within competences whilst managing the demands of unplanned aspects of the role where appropriate.
  • To participate in regular supervision.
  • To contribute to the process of induction for new team members and demonstrates duties to others as required.
  • To identify own training/educational needs as part of the supervision and appraisal process in conjunction with registered practitioners. Attains and maintains an agreed level of competence through ongoing training and development
