Integrated Care Pharmacist (Discharge to Assess Pathways)

This is an unique and exciting opportunity to lead on the development of an Integrated Home First Medicines Optimisation Service to improve safe transfer and use of medicines across clinical interfaces; to meet all legal requirements and the Good Practice Standards prescribed by the Care Quality Commission (CQC) and other relevant agencies; to support safe administration of medicines in community settings; to meet the recommendations published by NICE NG 67 "Managing medicines for adults receiving social care in the community.

Main duties of the job
JOB PURPOSEIn line with the Trust Medicines Optimisation Strategy, developing and delivering a clinical pharmacy service for designated patient cohort to improve patient outcomes, enabling a range of multidisciplinary practitioners within the team to optimise the use of medicines within their scope of practice. The designated patient cohort will primarily include patients requiring care within the Home Based Rehabilitation and Assessment Services (Home First, D2A pathway 1)Develop a single meds management framework for medicines optimisation within Home First Services. Promote the safe, rational and cost effective use of medicines by working closely with medical and non-medical health and social care professionals colleagues from all disciplines to share good practice and change prescribing behaviours where necessary.Lead on all medicines aspects of patient care to optimise medicines utilising tools such as STOPP-START, STOMP, 7 STEPs in response to the National Overprescribing Review around polypharmacy and overprescribing, reduce medicines related risks or adverse effects and improve adherence and empower patients.Assist the Deputy Director of Pharmacy in the management and strategic development of patient-centred clinical pharmacy services across the Discharge to Assess Pathways; Co-ordinating and leading interventions to reduce medicine related problems by developing care plans in agreement with patients and other health and social care professionals.

1. To develop and deliver high quality patient centered clinical pharmacy services within Home First Services including clinical, governance and operational functions. Implementing collaborative medicines optimisation on admission in to D2A pathways.
2. Develop robust processes and systems to deliver individualised medicines optimisation, as well as the implementation of safe administration and handling processes for the various staff involved at all stages of medicines management. This includes medicine reconciliation on admission in to the D2A home First services, scope and implement a solution that will enable electronic issue of Medication Administration Charts to aid administration. This requires engagement with a range of stakeholders including but limited to Track and triage teams, Home First Clinical and Operation Teams
3. Identification of appropriate cohorts of patients for medication review admitted in to D2A Home first Services, including patients on complex medication regimes and possibly high-risk medication regimes Carry out and support procedures for medicines reconciliation, medicine adherence and self- administration to ensure pharmaceutical care is appropriate in community care and home settings. Structured medication reviews targeted to those people with the most significant health needs will be undertaken in coordination with acute Trusts, GPs and practice-based clinical pharmacists to reduce overmedication, re-admissions and support meaningful savings in unnecessary prescribing costs
