Job summary

Providing a high quality Physiotherapy service to adults with learningdisabilities who are referred to the Shropshire Community LearningDisabilities Team (CLDT).

You will provide a highly specialist Physiotherapy Service to adults with complex learning and physical disabilities; undertaking clinical diagnosis, assessment and physiotherapeutic management and interventions.

Are you looking for an exciting new opportunity? We are looking for an experienced, enthusiastic and motivated band 6 Physiotherapist to join our well established, friendly and supportive Community Learning Disability Team covering the Shropshire, Telford and Wrekin Locality. This position is part time (22.5hrs) on a permanent basis.

You will be part of an experienced multidisciplinary team including Psychology, Psychiatry, Community Nursing, Speech and Language Therapy and Occupational Therapy. Our team works with Adults (18+) with a diagnosed learning disability and a critical and substantial health Need.

This is a dynamic post that will require you to work closely with multi-disciplinary/agency teams to provide a highly specialist Physiotherapy Service to adults with complex learning and physical disabilities; undertaking clinical diagnosis, assessment and physiotherapeutic management and interventions. Interventions will include physiotherapeutic handling, postural management, respiratory support via blended working and some face to face clinics.

JOB PURPOSE:

  • To provide a highly specialist Physiotherapy Service to adults with complex learning and physical disabilities; undertaking clinical diagnosis, assessment and physiotherapeutic management and interventions.

KEY DUTIES AND RESPONSIBILITIES:

1. Carry out comprehensive, evidence based physiotherapy assessments for clients with complex needs. Interpret clinical and non-clinical information using clinical reasoning skills to accurately diagnose and develop specific treatment plans. Provide intervention and/or recommendations for appropriate intervention for a small complex caseload. Interventions will include physiotherapeutic handling, postural management, respiratory support via blended working and some face to face clinics

2. Use high level clinical reasoning skills to identify, evaluate and prioritise clinical need.

3. Provide physiotherapy assessments and expert advice to other professionals, clients, carers and family to meet clients individual care needs in relation to physiotherapy treatments / interventions.

4. Ensure that clients and their families and/or carers are involved in all aspects of physiotherapy input. Negotiate with clients and carers to identify and formulate client-centred goals and demonstrate ongoing evaluation of client progress using agreed outcome measures.

5. Ensure that equipment identified during assessment is ordered and provided as required including seating, positioning and mobility equipment, orthotics and wheelchairs. Ensure that items of equipment are safe to use and that those using the equipment are competent to do so.

6. Work collaboratively with the multi-disciplinary team, clients, advocates, paid carers and family carers to provide a model of intervention that addresses the clients individual needs.

7. Supervise and provide specialist physiotherapy advice to other members of the multi-disciplinary team in relation to complex cases.

8. Triage and prioritise urgent, unexpected and planned referrals, adjusting caseloads and allocations to meet revised needs.

9. Gain and document consent or Best Interest decisions for physiotherapy interventions.

10. Actively participate in and chair when required Multi-disciplinary team meetings, including Professionals Meetings, Care Programme Approach, Best Interest meetings and safeguarding.

11. Write detailed reports and programmes on clients progress and rehabilitation in line with identified needs.

12. Develop outcome measures for and with service users and their carers. Monitor service users progress as necessary and discharge them from the service as appropriate.

13. Design, develop and provide training in physiotherapy interventions to paid carers, families and other communication partners, service users, other professionals within and outside the immediate multidisciplinary team.

14. Identify, assess and manage clinical and other risks. Record near misses and actual events through the incident reporting and safeguarding processes.

15. Actively participate in the transition process to other services and / or referral to other professionals or agencies as required.

16. Delegate programmes and work to Assistant Practitioners.

17. Work closely with other physiotherapists in the wider Adult Learning Disabilities team to develop the service and provide seamless transition between services.

18. Carry out audits to improve the service and compliance with local and national standards.

19. Add to the evidence base related to this specialist area of physiotherapy.

20. Share responsibility for security, care, safe use and maintenance of medical devices and ensure standards of infection control are maintained.

21. Work with and influence senior management in the development of services related to physiotherapy provision for the client group.

22. Maintain continuing professional development log.

23. Undertake appropriate statutory training, including an induction to the trust.

24. To attend Clinical Effectiveness Groups relevant to role and play an active part in their development.

25. Maintain up to date safe practice and to be registered with the Health and Care Professions Council and the Chartered Society of Physiotherapists.

26. Explain the role of physiotherapists to families, paid carers, public, service users, students, volunteers and other professionals.
