Main duties of the job
The Clinical Nurse Practitioner in the IST will work as part of a multi-disciplinary team, for people with learning disabilities, complex needs, behaviour that challenges and related mental and physical health issues. The service provides person centred care, PBS assessment, and intervention between 9am and 5pm (Monday to Friday) but is flexible, responding to individual needs providing support if required over the 24-hour 7-day week period. The post holder will be responsible for ensuring the clinical supervision of junior post holders in the IST team as delegated by the Clinical Lead for IST Shropshire.
The post holder will maintain high standards of clinical and professional practice in relation to the healthcare needs of people with learning disabilities, behaviour that challenges and mental and physical health needs, working closely with families, carers and advocates.
The role will involve collaborative working and ensure robust partnerships between, the IST and Specialist Community Learning Disability Teams (CLDT's), inpatient services, mainstream, statutory and independent sector services.
