Main duties of the job

Our Community Nursing team is admitting patients with a more complex acuity than ever before with increasingly specialist treatment and care being provided by highly skilled staff. We are recruiting dedicated, compassionate nursing staff to further enhance the high quality patient experience we strive for in our community team.

This post would suit either an experienced or a newly qualified Registered Nurse who wants to be part of a team with a culture of continuous improvement. We encourage and actively develop our nurses to progress, with each nurse in our team having a link role of supported specialist interest which offers opportunities to develop specialist knowledge with further education and training. Individualised professional development, regular supervision and mentoring/preceptorship will be provided, ensuring that the post holder feels fully supported and registration requirements met.

Job responsibilities

We are a well-established, supportive and friendly Community Nursing team excited to be offering a great opportunity in Community Nursing. The post will be aligned to the Stafford South Primary Care Network delivering nursing care across the areas of Brewood and Coven (South Staffordshire), Penkridge and Rising Brook. Full or part time hours are available.

The successful candidate will work within a small caseload led by an experienced Senior Community Nurse or District Nurse with overarching support from our in-team Clinical Lead. The successful candidate will be able to complete effective assessments, plan and deliver evidenced base care and enjoy working autonomously in our diverse community.

The post holder will:

  • Have a passion for working with patients in their own homes
  • Contribute to the development of evidence based services
  • Demonstrate a commitment to Continuous Professional Development
  • Be able to meet the travel requirements of the role by having a driving licence and use of a car
  • You must be a qualified nursing practitioner with current NMC registration.
  • Previous experience of working in the community is not an essential requirement.

Within our team we have a shared approach to working with patients, encouraging self-care utilising individualised care planning to meet our patients needs. The role will involve working closely to develop effective partnerships with other agencies, GP partners, carers, families and other specialist teams to ensure that quality nursing care is delivered at the highest level.
