Band 5 - Band 6 Developmental Post - Community Mental Health Nurse

Main duties of the job
- Working in the Memory and Dementia Team delivering high-quality clinical service to service users and their families.
- Managing a complex caseload and being responsible for clinical and caseload supervision of other team members where identified.
- Operating as a care coordinator for a defined caseload of service users
- Delivering therapeutic interventions, following an agreed care plan, to an identified group of people experiencing mental health-related difficulties.
- You will be responsible for the assessment of care needs and contributing to; the formulation of the person's needs and the development of a relevant plan based upon the person's negotiated goals and relevant evidence based interventions.
- You will, in conjunction with the multi-disciplinary team, provide nursing interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical

The team is multi-disciplinary, consisting of Psychologists, Mental Health Nurses, Occupational Therapist, Clinical Support Workers, Care Home and Education Support Nurses with input from Medics and administrative support.
Applicants should possess the knowledge and skills to enable them to work in the older adult mental health specialty as well as with people and their carers experiencing all types of dementia with challenging and complex needs.
The Memory and Dementia will provide you with a supportive environment in which to fully utilise and develop your skills and knowledge in working in a challenging community setting as part of a truly holistic system of health and social care.
Things that you will have:
- A relevant nursing qualification and registration RMN
- Experience of working with people who have difficulty in engaging with services and have complex mental health needs
- Strong team-working skills
- A high level of self-motivation and a flexible approach
- A positive attitude and be naturally caring and compassionate
- A commitment to high-quality care and values and evidence-based practice
- Excellent teamwork skills, and professionalism
Working primarily between Monday and Friday. Access to car during working hours is required
