Main duties of the job

Be competent as appropriate to the requirements of the role. Demonstrate at all times the Partnership Trust Values of: "Putting Quality First, We Care About People and We Take Responsibility."Be a qualified health visitor or school nurse and have a detailed professional and clinical knowledge base. They will work autonomously and collaborate with partners in other services involved in locality community initiatives and community development in order to promote and facilitate the shift of provision of health services offered from the 0-19 service to be community focused where it is appropriate to do so and where they are not currently.Act to support best practice of service provision with partners and act as a resource regarding community initiatives for the locality 0-19 team.Act as a named practitioner in relation to communication with partners in relation to health initatives from the 0-19 service operated within the local community.To plan, deliver and evaluate training and update study days to service users and partners where this is required to deliver community focused initiatives.Be skilled in profiling health requirements of the population within a district and map current services available to meet these needs, and identifying areas for development, where they can support increased provision working with partners, to meet identified community health related need. Demonstrate initiative and be creative in finding solutions to problems.
