Job summary
Integrated Offender Management (IOM) was introduced in 2009. Its aim is to bring a cross-agency response to crime and reoffending threats faced by local communities. Service users will be jointly managed by Police, Probation and other partner agencies; to reduce reoffending and make communities safer. The nursing role has support and supervision from the Forensic community team and a partner I.O.M nurse in North Staffordshire. The role sits in South Staffordshire police stations and aims to:

Offer mental health assessment, support and treatment for the Integrated Offender Management I.O.M population

Refer to primary mental health, secondary mental health services where care coordination is needed.

Develop close partnerships, training and support across Staffordshire police, Probation and the third sector.
Main duties of the job
This role sits within the criminal justice teams and f ills the gap between short term criminal justice services such as Liaison and Diversion and Primary and secondary mental health Teams.
The main duties include:
- Assessment and care planning in conjunction with service, carer, probation and Staffordshire Police.
- Evaluation of individual treatment plans
- Participate in and contribute to weekly IOM case management meetings, ensuring that colleagues from other agencies are kept fully informed of the status and progress of IOM individuals on caseload.
- Risk management
- Handover to other services as needed.

Band 6 community mental health nurse role
37.5 hours per week across Monday to Friday 9- 5 (flexible pattern)
Agile working: Mixture of home working, community visiting and some time at South Staffordshire Police stations.
