Job summary

The successful applicant will be able to function confidently within the multi-disciplinary team and will possess excellent communication and interpersonal skills. They will be self motivated and will value highly the principles of the recovery approach and the engagement of service users in planning their care.

Additionally applicants should be dynamic, committed to providing care which is evidence-based and willing to undertake further training to meet the needs of the client group. A commitment to continuing professional development is essential, as is a flexible and dynamic approach to care. Due to the sometimes stressful nature of this work a commitment to clinical supervision is expected.

We would welcome applications from both newly qualified and more experienced nurses qualified in either Learning Disability or Mental Health.

Job Purpose Summary

  • To work in a low or medium secure setting
  • Provide high quality clinical care that is evidence-based, open to monitoring and evaluation, and clinically effective
  • Working in low or medium secure settings with predominantly mentally disordered offenders or patients who may have a diagnosis of a learning disability, autism or ADHD and may have an offending history.

Midland Partnership NHS Foundation Trust provides a comprehensive range of mental health and related services to the population of Staffordshire and Shropshire.

Ellesmere House is a 12 bedded low secure Learning Disability unit. We cater for individuals who have complex needs and include primarily a learning disability but may also possess autism and/or mental health issues. The client group may also but not exclusively require partnership working with agencies such as the Criminal Justice System and Home Office.

Ellesmere has a full range of specialized provision including our own consultant, psychology, social worker and an enhanced OT/activities coordinator provision. We are an integrated team and very much work together as a team. The nursing team is a committed group as are the entire team overall as a unit.

The Hatherton Centre is a 47-bedded medium-secure facility incorporating an intensive care ward, acute assessment ward and rehabilitation wards.

The Clee unit (in Shropshire) is a 32 bedded low secure mental illness unit (The Clee Unit) which includes a rehabilitation service and an acute assessment service and a multi-agency community service, liaison scheme, out-patient facilities and court assessment and engagement schemes.

Organisational Position

Through others, to the person with overall responsibility for the ward or department in which the nurse works. Take charge of ward in the absence of Charge Nurse/Sister or Ward Manager.

Scope and Range

To work within the Forensic Directorate Nursing Services.

Main Duties and Responsibilities

    • To act as key worker or CCR to an individual or group of service users, coordinating all aspects of care in accordance with the Care Programme Approach (CPA).
    • Assessment of patients individual care needs.
    • The development of programmes of care.
    • Implementation of agreed programmes of care.
    • Evaluates effectiveness of programmes of care.
    • Ensures compliance with NMC Code of Conduct
    • Measures quality of care by the use of appropriate tools.
    • Implements Mental Health Act (1983) appropriately.
    • Implements local and national policies.
    • Promotes and maintains effective relationships with other disciplines.
    • Promotes and maintains effective relationships within the ward team.
    • Takes charge regularly of shift in ward environment.
    • Acts as primary nurse for Clinical Team embracing multi disciplinary working.
    • Identifies training requirements of self and junior staff.
    • Maintains an up to date level of professional competence.
