Main duties of the job

If you are passionate about improving services for people living with learning disabilities and/or autism and have good knowledge of the Learning Disabilities and Autism/Transforming Care Agenda. We would love to hear from you!

As part of delivering the Transforming Care/LD&A Agenda, Midlands Partnership Foundation Trust (MPFT) are leading on the new all-age Dynamic Support Register (DSR) for people with learning disabilities and or autism who are at the highest risk of a hospital admission across Staffordshire.

You will drive forward the work required to continually develop the DSR; establish robust processes and governance arrangements to support its use; promote its aims and benefits; and successfully embed it across South Staffordshire.

You will link with all relevant local partner organisations within the Integrated Care System (ICS) including; Local Authorities, Health Trusts, Education and Police working alongside the DSR Lead in the North Staffordshire. Ensuring the ongoing development of the DSR continues to be co-produced with experts by experience.

The DSR helps the system to provide increased vigilance and improved strategic planning for those at high risk of placement breakdown and/or hospital admission, ensuring proactive monitoring and support reducing the risk of inappropriate admission.

The DSR forms an integral part of the County's admission avoidance initiatives and improves the system's proactive responses to support people when they are in most need.

Where all agencies and services involved in commissioning and delivering care and support for people with a learning disability and/or autism in Staffordshire and Stoke-on-Trent work together in the continued development of the DSRs comprehensive and proactive digital database. Helping to identify people at most need of support to reduce the likelihood of unnecessary or inappropriate admission to mental health or specialist hospitals.

With close links to the Transforming Care Programme (TCP)/LD&A the DSR provides a collaborative forum for communication where a persons support needs can be reviewed and supported appropriately.

Essential

  • Qualified Health / Social Care Practitioner or equivalent degree level qualification (or Masters level qualification) within Health or Social care
  • Training in project management (or willingness to undertake this)

Desirable

  • Demonstration of continuous professional development in relation to people with learning disabilities and or/autism and people who require support with their mental health and/or complex behavioural needs
  • PRINCE2 Qualification in Project Management

Experience

Essential

  • Significant post qualification experience of working at a senior level in a Mental Health, Wellbeing, Education, Learning Disability and/or Autism Service.
  • Relevant Local Authority/NHS/Public Sector/Third Sector experience
  • Experience of working across other organisations and with other agencies including health and social partners
  • Experience of handling sensitive and confidential data and knowledge of data protection and data security where relevant
