Job summary

We are looking to recruit a band 5 physiotherapist to work within our existing Clinical Assessment Team (CAT) team based in Stafford covering the Stafford and surrounds district.The core remit of the service is to provide Rehabilitation, Reablement and Recovery to those individuals who have been discharged from hospitalwith the emphasis of getting them back to their previous level of independence within their own homes.

Main duties of the job

You will be working closely with qualified and non-qualified therapist's and with the remit of supporting individuals who have been recently dischargedfrom hospital back home with Home first reablement support or to a rehab bed in a local care home. Your role will be to maximiseindividuals' independence and reduce the need for long term services.

The post holder will be competent in the key elements of the career framework levels 1 to 6 (skills for health). The post holder will be responsible for a defined population of patients within the Community and or Hospital setting and will work with the Team Leader by conducting delegated tasks instrumental in team management and care co-ordination. The post holder will be skilled in the assessment of complex care needs and in the development, implementation, and evaluation of treatment plans.The post holder will be a senior Physiotherapist and have an understanding of detailed theoretical and practical knowledge. They will have a focus in one of the following areas Rehabilitation, Integrated Community, Community Intervention and Enablement. They will work with specialist OTs /team leaders, for day to day service delivery. They will demonstrate initiative and be creative in finding solutions to problems. They will have supervisory, training & leadership responsibilities where appropriate and will have some responsibility for team performance & servicedevelopments.
