Job summary

Working within clinical areas the post holder will facilitate a safe patient discharge providing a high quality service in line with Trust standards. They will be able to communicate sensitivity and effectively with a wide range of people including patients, families, colleagues and agencies external to the Trust to help discharge patients.

Main duties of the job

The Discharge Co-ordinator will manage patient flows within a defined area and will be responsible for prioritising work, reviewing working practices to ensure timely and efficient patient access to the Trusts services, delivery of the patient administration pathway from the decision to admit the patient and throughout the inpatient process.

Job responsibilities

To provide a high standard of confidential, effective and accurate services that facilitates effective and efficient discharge.

Communicate respectfully sensitively and effectively with patients, relatives, colleagues and all other healthcare professionals.

Respect the working environment, to be responsible for ones own acts and omissions in relation to Health & Safety.

Respect confidentiality in line with national, Trust and local policies and guidelines.

The post-holder is expected to be honest, fair and consistent in their approach and to accept responsibility for ones own achievements, acts and omissions.

The post-holder will be expected to be a good team player, with the ability to work flexibly in a diverse and demanding environment to meet the evolving needs of patients and the department.

Support and guide colleagues who have less experience or during their induction period.

Responsible for appropriate use of equipment within the working environment.

A willingness to undergo all training requirements necessary for the post.

Undertake duties on other wards or departments as and when required by service demands or patients need, recognising and working within their own clinical limitations.

Act in such a way that safeguards the health and wellbeing of children and vulnerable adults at all times. Is familiar with and adheres to the LTHT safeguarding policies.

Raise concerns with regards to risk, danger, malpractice or wrong doing by following the LTHT Whistleblowing policy and supporting processes.

Complete mandatory training.

Service Delivery

Working with the clinical ward teams facilitate seamless timely discharge or transfer of patients to an agreed discharge plan.

Document discharge plan in PPM+ following the board/ ward round in order to keep the patient information up to date.

Work closely with internal clinical teams and external agencies to support effective patient discharge.

Have an understanding of the Reason to Reside Criteria (R2R) and the codes and descriptions explaining what a patient is waiting for to be discharged from hospital.

Promptly alert the Nurse in Charge/discharge facilitator of any delays or difficulties within a patients discharge plan.

Promptly alert Nurse in charge to difficulties in patient flow, placement or when capacity issues arise within their responsible ward areas.

Collect and maintain accurate and up to date information regarding patient flows (acute and elective admissions, discharges, transfers, delays).

Ensure all patient and carer enquiries are dealt with in a friendly and professional manner.

Participate and contribute to Multi-Disciplinary Team meetings or board rounds to ensure the full team understand individual patient discharge plans

Communicate with patients and families their discharge plan ensuring they receive the most up to date information.

Information Management

Collate and enter data, on the relevant Trusts administration system, regarding patient discharge in an accurate and timely manner in accordance with local and Trust policies.

Provide reports as required regarding transfers and discharges.

Maintain accurate records of patients waiting for investigations and or treatment and provide reports as necessary regarding numbers of patients waiting, current waiting times and outcomes.

Maintain accurate records of what patients are waiting for to go home or to another place of care .

Monitor administrative processes to ensure that they meet the needs of the patients and the service.

All staff are responsible for working with their colleagues to maintain and improve the quality of services provided to our patients and other service users. This includes complying at all times with the Leeds Teaching Hospitals NHS Trust Policies, including Health and Safety policies, in particular by following agreed safe working procedures, and reporting incidents using the Trust Incident Reporting system.

The jobholder must comply at all times with the Leeds Teaching Hospitals NHS Trust Infection Control policies, in particular by practising Universal Infection Control Precautions. Hand hygiene must be performed before and after contact with patients and their environment.

The jobholder must comply with all policies and procedures designed to ensure equality of employment and that services are delivered in ways that meet the individual needs of patients and their families. No person whether they are staff, patient or visitor should receive less favourable treatment because of their gender, ethnic origin, age, disability, sexual orientation, religion etc.

The Trust has a statutory duty to involve patients and public in evaluating and planning services. All staff have a responsibility to listen to the views of patients and to contribute to service improvements based on patient feedback.
