Job summary
Working within clinical areas the post holder will facilitate a safe patient discharge providing a high quality service in line with Trust standards. They will be able to communicate sensitivity and effectively with a wide range of people including patients, families, colleagues and agencies external to the Trust to help discharge patients.
Main duties of the job
The Discharge Co-ordinator will manage patient flows within a defined area and will be responsible for prioritising work, reviewing working practices to ensure timely and efficient patient access to the Trusts services, delivery of the patient administration pathway from the decision to admit the patient and throughout the inpatient process.
