2. JOB PURPOSE
Our service delivers highly specialist diabetes care to patients with Type 1 diabetes, complex Type 2 diabetes and provides in-patient care.
We cover the diabetes centres at St Jamess and Wharfedale Hospitals and in- patients at St Jamess, Leeds General Infirmary and Chapel Allerton.
We are looking for a diabetes specialist nurse who is enthusiastic and committed to the provision of excellence in diabetes care and wants to further develop their knowledge and skills in this field. The successful applicant will strengthen our diabetes specialist nursing service, providing expert advice to patients, education to staff and working with the Trust Think Glucose delivery team.
There will be excellent opportunities to rotate between both inpatient and outpatient nursing services. Training will be given to provide expert care in highly specialist clinics to include; antenatal, preconception, renal, foot, young adult, pump and DAFNE.
The jobholder is responsible for the management of a defined caseload. The jobholder carries responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care. The jobholder will provide specialist nursing care and advice to a specific patient group and will contribute to the development of the speciality service. The jobholder will ensure cost effective use of resources.
Our service provides a 7 day a week diabetes specialist nursing cover across LTHT. The jobholder would be expected to work flexibly with colleagues to provide this cover on a rotational basis.

Band 7 Diabetes Nurse Specialist - Team Leader

Clinical Support workers
The post holder will work within a team of Nurse Specialists and allied health professionals.
5. KNOWLEDGE, SKILLS AND EXPERIENCE REQUIRED
Qualifications
Registered Nurse (Level 1)
Recognised teaching/assessing qualification
Post registration qualification in specialty
Experience
Significant experience working as a Band 5 Staff Nurse or equivalent post registration experience of which there has been substantial experience gained within the relevant specialty area, during which time the following experience should have been gained:

9. PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY

Clinical care delivery
- Provide a clinical lead and role model to nursing staff by delivering high quality care.
- Undertake clinical procedures where dexterity and accuracy are required

Responsibilities for patient and client care
- Assess, plan, implement and evaluate care and treatments using specialist clinical knowledge and clinical skills
- Organises own clinical workload.
- Receives supervision of clinical care and outcomes from nurse specialist team leader.
Freedom to act
- Refer patients as appropriate to other members of the multi-disciplinary team
- Make clinical assessments and treatment decisions within clearly defined policies and protocols
Communication
- Maintains accurate clinical and other records using the Leeds Teaching Hospitals Trusts documentation.
- Support patients and their families, communicating sensitive condition related information, ensuring they receive required information to enable them to participate in their care delivery
- Liaise with Community Services (as appropriate) to ensure seamless provision of care delivery
Education and Training
- Provide training and education of learners and other junior staff
- Actively participate in the development of own personal development plan and performance review
- Provide education and training to patients and their carers.
- Keep up to date with developments, advances within the specialty
Leadership and management
- Investigate and respond to accidents, complaints, untoward incidents and other significant events
- Maintains record of workload and performance monitoring information and reviews this on a regular basis with team manager.
- Work with Team Leader and/or Matron to promote the development of nurses undertaking the CNO Ten Key Roles as considered appropriate
- Under the direction of the Team Manager participates in or leads the development of specific elements of nursing practice, reflecting the CMT and Nursing Directorate business plans

- Participates in the setting, monitoring and improvement of standards and quality of patient care, including those standards defined in the National Benchmarks for Essential Care

Health and Safety/Risk Management
All staff are responsible for working with their colleagues to maintain and improve the quality of services provided to our patients and other service users. This includes complying at all times with the Leeds Teaching Hospitals NHS Trust Policies, including Health and Safety policies, in particular by following agreed safe working procedures, and reporting incidents using the Trust Incident Reporting system.
Infection Control
The jobholder must comply at all times with the Leeds Teaching Hospitals NHS Trust Infection Control policies, in particular by practising Universal Infection Control Precautions. Hand hygiene must be performed before and after contact with patients and their environment.
