Clinical Nurse Specialist (Parkinsons disease and Movement Disorders)

An exciting opportunity has arisen to work within the integrated specialist Parkinson and movement disorders nurse team based at the LGI within the Regional Neurosciences service and Neurology Multi-Disciplinary Team.

This is a part-time role of 22.5 hours.

This is a rewarding and challenging role caring for patients with complex long term neurological conditions.

Main duties of the job

Main duties of the job

The role of the specialist nurse is to monitor a defined patient caseload supporting with their diagnosis and on-going care and symptom management in all stages of Parkinson disease (PD) and movement disorders including Progressive Supranuclear Palsy, Multi System Atrophy, Cortico Basal Degeneration and Lewy body Dementia.

The specialist nurses provide advice and support to patients and carers, health care professionals within LTHT and the community including GPs, care home staff, community nursing and therapists. We work with Nurse advisors from pharmaceuticals companies to provide advanced therapies for complex PD patients.

Overview of your organisation

We are a team of four specialist nurses based at the LGI and working within LTHT and the community. We assess patients in outpatient clinics within the trust, in-patient ward reviews, telephone clinics and home visits. A full UK driving license would be required to travel to hospital and community clinic settings, home visits at patient homes and care homes.

2. JOB PURPOSE

The Jobholder is responsible for the management of a defined caseload. The jobholder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care. The jobholder will provide specialist nursing care and advice to a specific patient group and will contribute to the development of the speciality service. The jobholder will ensure cost effective use of resources.

3. JOB DIMENSIONS

The jobholder will work alongside the current Band 7 and 6 nurses within the team. We provide information, education, advice and support for those patients with a new diagnosis throughout their disease trajectory. This can be undertaken via the telephone, home visit, within patient groups, inpatient contact or outpatient clinic settings. The role requires good communication / organisational skills and close liaison with all members of the Clinical/MDT Team.
