Job summary
Building
on the success of the MS Trusts pilot programme, we are looking to recruit an
Advanced MS Champion within a well-established MS Team at LTHT.
This
is an exciting new post to work seamlessly with patients across specialist
and community care environments in Leeds.
Main duties of the job
We
are seeking a Band 7 MS specialist practitioner who has highly specialist
knowledge and understanding of the needs of people living with advanced MS
and their families. They will have highly developed clinical expertise,
knowledge and skills enabling them to plan, deliver and coordinate complex
care in routine and non-routine circumstances to ensure that the needs of the
person with advanced MS and their carer(s) are best served. They will work
autonomously with their own caseload.
The
role will involve instigating, implementing and evaluating service
improvement and changes in practice in response to feedback from patient
experience, research evidence, service level data, clinical and management
requirements. They will lead service level engagement with managers and
instigate and lead multi-professional meetings, acting as patient advocates
across organisational boundaries.

This
is a rewarding and challenging role that will be supported initially by the
MS Trust for 15 months leading to a permanent position within the MS Team at
LTHT.
