St James Adult Medicine Team are looking to recruit a motivated rotational
specialist clinical pharmacist. We
have a host of different rotations for you to learn from including Older
peoples Care, Respiratory and CF, General medicine, Acute medicine, ED, SDEC.

Main duties of the job
To
provide pharmaceutical care to a group of patients within a clinical area. To
provide cover for other pharmacists within the relevant Clinical Pharmacy
Team as appropriate. To support the work of the Lead Pharmacist for the area
by assisting in the production of drug-related policies, procedures and
financial reports, and by performing horizon scanning activities.

Job description
Job responsibilities
Provide
specialist clinical pharmacy service to patients and the multidisciplinary
team within the rotation, as part of a team of pharmacists working in that
speciality being professionally accountable for their actions.
Provide
drug-related advice and information to patients, consultants and other
health-care professionals within a Clinical Management Team.
Monitor
the clinical use of drugs in patients. Advising medical and nursing staff how
to optimize drug therapy. Reporting prescribing and administration near
misses, and risk management issues
Provide
education to healthcare professionals within the speciality
Participate
in the training of junior pharmacy staff within the speciality
Support
the Lead Pharmacist for the area with the production and implementation of
drug related policies and procedures, with the production of financial
reports and with horizon scanning activities.
Participate
in operational service commitments to dispensaries, Medicines Information and
aseptic services
The
post holder will be required to work evenings, weekends and Bank Holidays on
a rotational basis and participate in the on-call from home pharmacists rota.
The
post holder will be required to participate in the core professional
commitments.
To
