Job summary

We have an exciting opportunity for a Specialist Clinical pharmacist to join our dynamic clinical pharmacy team at Leeds General Infirmary. We provide Medicines Optimisation to patients across a range of medical and surgical specialties including neurosciences, cardiology, critical care, and major trauma as well as highly specialist areas such as hand transplant services, spinal surgery, cardiac surgery and a hyper-acute stroke unit. We are passionate about developing our team and our services and actively review our skill mix and the roles of all the pharmacy staff within our team.

Main duties of the job

The post holder will rotate annually amongst the specialities which allow you to further your knowledge base while becoming an integral member of the wider multidisciplinary team. You will be supported in your development by a team of Advanced Clinical pharmacists and will receive training to carry out your role. You will assist the team in developing and implementing improvements in patient care and experience. You will also help to supervise undergraduate students, rotational trainees and foundation level pharmacists. The rotations are designed to steer you through the RPS Advanced Pharmacy Framework.

This post is suitable for dedicated and enthusiastic pharmacists with clinical pharmacy experience and a diploma. We would be looking to support you to gain the non-medical prescribing qualification if you are not currently a prescriber.

Job responsibilities

JOB PURPOSE

To provide pharmaceutical care to a group of patients within a clinical area and rotate to a different speciality every 6 to 12 months. To provide cover for other pharmacists within the relevant Clinical Pharmacy Team as appropriate. To support the work of the Lead Pharmacist for the area by assisting in the production of drug-related policies, procedures and financial reports, and by performing horizon scanning activities.

JOB DIMENSIONS

Provide specialist clinical pharmacy service to patients and the multidisciplinary team within each rotation, as part of a team of pharmacists working in that speciality being professionally accountable for their actions.

Provide drug-related advice and information to patients, consultants and other health-care professionals within a Clinical Management Team.

Monitor the clinical use of drugs in patients. Advising medical and nursing staff how to optimize drug therapy. Reporting prescribing and administration near misses, and risk management issues

Provide education to healthcare professionals within the speciality

Participate in the training of junior pharmacy staff within the speciality

Support the Lead Pharmacist for the area with the production and implementation of drug related policies and procedures, with the production of financial reports and with horizon scanning activities.

Participate in operational service commitments to dispensaries, Medicines Information and aseptic services

The post holder will be required to work evenings, weekends and Bank Holidays on a rotational basis and participate in the on-call from home pharmacists rota.

PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY

Provide pharmaceutical care to a group of patients in various specialities on a rotational basis. Be responsible for ensuring that the legal and statutory requirements in relation to medicines are met and ensure that the delivery of pharmaceutical care to patients in the defined clinical area meets both national and local standards.

Provide professional pharmacist cover on a sessional basis to inpatient and outpatient dispensaries, Medicines Information, Aseptic services and Mental Health, in order to meet legal requirements.

Participate in the dispensing and checking of clinical trial materials

Participates in documenting of prescribing and administration near misses (interventions)

Assist clinical colleagues in the development and implementation of policies and procedures for the informed, economic use of medicines

Assist clinical colleagues with horizon scanning activities for new treatments and products in the relevant area and with submissions to the Drugs and Therapeutics committee

Assist clinical colleagues in the provision of accurate financial and risk-management data to the appropriate Clinical Management Team for the speciality

Participate in the provision of Education and Training activities for all pharmacy staff

Participate in research and development programs and audit within pharmacy services

Receive prescription payments from patients where appropriate.

Participate in the departments late night and weekend working arrangements.
