1. BACKGROUND

This is a 12-month clinical trust grade post in Leeds Teaching Hospitals commencing August 2023 and designed to provide comprehensive experience in all aspects of critical care medicine. This post is suitable for applicants wishing to gain or develop their experience in Intensive Care Medicine. Typical applicants undertake this position with a view to gaining tertiary centre experience in ICM whilst considering their career plans with respect to either entering the national training programme or the CESR pathway to become a fully trained consultant in Intensive Care Medicine or Anaesthesia.

Leeds General Infirmary (LGI) provides tertiary surgical and medical services for the large population of West Yorkshire as well as some centrally commissioned services covering all of Yorkshire. Our services include major trauma, cardiology, stroke and neurology, cardiac surgery, neurosurgery, vascular and ENT.

The successful applicants will join an existing team of Intensive Care and Anaesthetic trainees on the rota, supported by a wider team of Medical SHOs and Allied Health Professionals. There is extensive support from the 15 Critical Care Consultants and this post provides an excellent environment to experience a complex, diverse and busy intensive care environment.

There is also occasional potential to cover shifts at Saint James University Hospital, which is a regional centre for oncology, hepatobiliary surgery and thoracic surgery, as well as a liver and kidney transplant centre. SJUH also provides the acute general medical and general surgical services for Leeds.

2. OBJECTIVES OF THE POST

1. To work with other critical care colleagues in the provision of a comprehensive

critical care service to Leeds Teaching Hospitals NHS Trust.

2. To develop and / or consolidate expertise and experience in intensive care

medicine.

3. To support service plans for the specialty, taking account of the changes in

clinical practice and new technologies available.

4. To participate in and progress Clinical Governance.

5. To develop specialised areas of interest eg Simulation, Teaching and

Research.

3. REQUIREMENTS OF THE POST

3.1 General Training and Qualifications

The appointee must have full registration with the General Medical Council and have as a minimum completed Acute Care Common Stem (ACCS), Core Anaesthetic Training (CAT), or equivalent. Applicants must provide information regarding their status from the GMC and/or relevant Royal College at the time of application, in order for their application to be progressed further.

3.2 Experience

The appointee must have sufficient experience that they are able to competently manage the acutely deteriorating patient who requires intensive care support. This includes management of the airway of an unconscious patient.

3.3 Research

Although this is a clinical post, the appointee will have the opportunity to actively participate in research projects. All main Anaesthetic and Intensive Care Medicine journals are available in the Hospital Library. The involvement of trainees in research is encouraged and supervision is provided. There is the opportunity to become involved in multi-centre randomised control trials and learn about becoming an associate Principle Investigator. PC access and other I.T. support is also available.

3.4 Personal Attributes

The appointee will have excellent communication skills in both written and oral English, and be able to work effectively as part of a team. They will have an understanding of current NHS management and Trusts and be aware of the responsibilities that a clinical fellow post brings.

3.5 Clinical Audit

The appointee will be expected to contribute and, where appropriate, take the lead in the following (e.g.):-

Mortality and morbidity meetings

Clinical audit

Development and application of agreed clinical guidelines

Simulation in Critical Care

In the discharge of these responsibilities the candidate will be expected to maintain and update his/her skills and knowledge through appropriate continuing medical education.

4. TEACHING

The Trust as a Teaching hospital recognises the importance of links with the University of Leeds School of Medicine for the Undergraduate teaching and all Clinical fellow and medical staff are required to provide an appropriate contribution to the undergraduate teaching programme.

The post holder will be required to participate in the informal teaching of medical students, nurses and operating department practitioners/assistants.

The post holder will have access to the Trusts extensive and spacious library in the Clinical Sciences Building that houses approximately 5,500 books and several hundred journals. Staff in this library will also arrange loans of books etc. from the Medical School Library and the National Lending Library based at Boston Spa.

5. JOB PLAN AND WORKING ARRANGEMENTS

Aim

This post is designed to provide clinical service to, and experience in, Intensive Care Medicine, primarily in Leeds General Infirmary, Leeds. Opportunities may be available to cover shifts at St Jamess University Hospital, where appropriate.

Objective

The post should attract those wanting to specialise in anaesthesia or ICM and it will provide extensive experience in ICM for a wide range of medical and surgical specialties. It will also give significant opportunities for clinical audit and research.

Summary of post

Critical care in LGI is provided in three areas: a 14-bedded General ICU (primarily caring for trauma, vascular and medical patients), a 16-bedded Neurosciences ICU and a 16-bedded Cardiac Surgical ICU. Duties include the management of all aspects of patient care in the critical care environment, as part of a team comprising trainees in anaesthesia and ICM, core trainees from other acute specialties, advanced nurse practitioners and consultants. A wide case-mix of patients is admitted to critical care in LGI:

Acute cardiology, neurology and stroke patients

Regional specialist units for:

o Interventional Cardiology, including Primary PCI, TAVI and Electrophysiology

o Stroke Thrombolysis and Thrombectomy

o Vascular Surgery - open and endovascular procedures

o ENT and Max-Fax Surgery

o Major Trauma

o Plastics and Reconstructive Surgery, including Hand Transplantation

o Cardiac Surgery, including Aortic Surgery and Adult Congenital Cardiac Surgery

o Neurosurgery, including endovascular treatments

The post is in line with the new junior doctor contract. The post will consist of night and weekend on call commitment. There are three rotas staffed by the trainee medical team and there is further support from allied staff and consultants.

The post will have ample time for clinical audit, research and teaching and will provide further opportunity to enhance personal development. There is widespread usage of point-of-care ultrasound and there will be supervision and time allowed to achieve all FUSIC modules and more advanced cardiac ultrasound if desired. The appointee will be expected and encouraged to take part in these activities as part of their one year fellowship.

There is ample consultant supervision and the opportunity to complete clinical assessments that could benefit the postholder in further training. We would look forward to supporting the successful candidate in the future if they wished to peruse CESR training in Leeds.

Regular weekly teaching sessions are held which includes core topics in ICM, M&M meetings, critical incident reporting and journal club.

The post is available on a whole time basis or as a job share for a period of one year. The post will be paid at the appropriate point on the Specialist Registrar pay scale.

Essential

• Basic Medical Qualification ( MBChB, MBBS or equivalent)
• Full GMC Registration
• Primary FRCA, MRCP, MCEM or equivalent

Desirable

• Completion of FRCA, FCEM, FFICM or equivalent

Other

Essential

• Physical - Medically fit to fulfil all aspects of the post, and to be able to respond quickly to emergencies, as necessary.

Experience

Essential

• Previous ICU experience at the level of a Core Trainee
• Completion of Acute Care Comment Stem (ACCS), Core Anaesthetic Training (CAT) or equivalent.
• ALS or equivalent.

Desirable

• Previous experience of working in an ICU as a Specialist Trainee
• ATLS or other advanced life support training

Clinical, Academic, and Personal Skills

Essential

• Understanding of research processes, and an ability to interpret research evidence and relate this to best clinical practice.
• A desire to teach undergraduate medical students and postgraduate doctors in small and large groups.
• Experience of participation in regular clinical audit. Knowledge of the audit process
• Well-organised and skilled in time management.
• Effective interpersonal skills including excellent communication skills in written and oral English.
• Demonstrate ability to relate to and work within a team

Desirable

• Evidence of the ability to carry out medical research as shown by published research in a peer reviewed medical journal.
• Training the Trainers; ATLS Instructor Courses or Generic instructors Course
• Presentation of Quality Improvement project.
