Job summary

A rare and exciting opportunity has arisen for a Full time Youth Worker to join our GIDs team based in Leeds Teaching Hospitals NHS Trust (LTHT) on a temporary, fixed time.

The post holder will assist in the development and delivery of a comprehensive support service for young people in both adult and paediatric GIDs service in LTHT. As part of transition providing support for those young people moving from paediatric services to adult care.

The post holder will be responsible for providing both group and individually tailored support, and lead on a wide range of appropriate recreational activities, to enable and encourage patients to socialize and access peer support. Some work will be provided in the evening or occasionally on a weekend therefore a flexible approach to working hours is desirable. There will also be times where the post holder will be working collaboratively with youth workers from other services in the trust on joint events.

Main duties of the job

The role of a youth worker will work to support young people to developed services and provision for the whole service. You must have experience of engaging with 11-25 year olds (preferably with health problems), be able to demonstrate excellent interpersonal and communication skills, as well as an ability to develop and maintain high clinical standards. The successful candidate will be joining a team who emphasises the importance of excellent patient care and continuously strives for improvement

Working as part of the multidisciplinary team (MDT), the postholder will support the transition from childrens to adult services and be expected to to liaise between relevant members of the MDT adult and childrens teams, primary care, mental health and other community-based services.

The youth worker will be part of an exciting project of developing an exciting service. They will work cross paediatric and adult service across two main hospital sites within LTHT where GIDs service service is based.

About us

The youth worker will work to develop provision and support transition in the GIDs service. The will be managed by the Youth Work Manager and CNS Transition nurse. The youth worker is part of the multidisciplinary team (MDT), the postholder will support the transition from childrens to adult services and be expected to to liaise between relevant members of the MDT adult and childrens teams, primary care, mental health and other community-based services

Job responsibilities

Working alongside the young people, use informal or formal assessment to establish the needs of this patient group.

Using these results, work closely with the healthcare team in establishing an appropriate response to support these young people throughout the service.

To provide and lead recreational facilities and youth work activities for young people in Leeds Teaching Hospitals Trust.

Ensure appropriate recreational materials and equipment are made available for young people within ward area and outpatient departments.

Encourage and supervise the interaction of young people with activities, with particular emphasis on group work and using motivational approaches, where appropriate.

To devise and develop ways in which peer group support can be achieved.

Ensure the appropriateness of available resources to be updated every quarter.

Work with young people to prepare them for specific activities and treatment plans.

To coordinate and supervise social support networks to encourage social interaction and support to young people in order to aid adjustment to diagnosis. Encourage peer support within this group.

To attend other units where young people may be cared for, and provide a link with external agencies where applicable.

To participate in fundraising processes, including suggestions for fundraisers, proposals of contributions to the service and accepting donations at charitable events.

To keep accurate records of financial outgoings and donated income for charitable trust to be monitored throughout the year.

Responsibility for ordering supplies and equipment as appropriate.

To ensure recreational and craft equipment is continually maintained on a weekly basis.

To supervise unqualified staff such as youth work, play specialist students and volunteers. Contribute to a learning environment for students and provide mentoring when required.

To coordinate creative and therapeutic activities and implement a range of strategies. Liaise with complementary therapies, music workshop, community and youth groups to contribute to these strategies.

To keep records of the numbers and types of patients receiving recreational services/input and to assess social and emotional capabilities before and after intervention.

To attend unit meetings e.g. clinical governance, action log, staff support regularly. To participate in a range of multidisciplinary meetings both within the trust and outside agencies regularly.

To observe patients and report any relevant information to multi-disciplinary team.

To provide an age appropriate environment in which young people can remain stimulated.

To assist young people to develop links and contacts with other young people with long term conditions.

To work collaboratively with other Youth Workers in LTHT and the UK, working alongside other professionals to offer an alternative view to working with young people.

Initiate and use risk assessments where required for activities on the ward, clinic and in the community.

Initiate, plan, co-ordinate, monitor and evaluate work which broadens access to social activities for young people.

To develop collaborative projects with similar services, organisations, and other agencies and communities eg, youth services, youth theatre, schools and youth groups.

To develop and maintain a close working relationship with people from the community as well as with those involved in other elements of the teams work.
